The <current-date-time> block now holds a single sentence without a
markdown heading. The sub-agent prompt keeps its heading.

perf(core): Move instance URLs out of the Instance AI system prompt 
Per-instance webhook and form base URLs made every instance's system
prompt unique, so no two instances shared a prompt-cache prefix. The
URLs now ride each user turn in an <instance-urls> block inside
<thread-context>.
