Internal follow-ups replay the user turn that already carries the
<instance-urls> block, so they skip it like the other ambient blocks.
Add service-level tests for both paths.

perf(core): Move instance URLs out of the Instance AI system prompt
...
Per-instance webhook and form base URLs made every instance's system
prompt unique, so no two instances shared a prompt-cache prefix. The
URLs now ride each user turn in an <instance-urls> block inside
<thread-context>.
