fix(core): Keep the skill system message within provider cache budgets (no-changelog)

The active-skill system message copied the caller's instruction provider
options. A copied cache marker is an extra breakpoint. For Anthropic it
could push a request to five breakpoints, and for OpenRouter, which reads
anthropic and openrouter cacheControl from system messages, nothing
counted it. The skill message now gets only the runtime's own Anthropic
marker, with the lowest priority in the breakpoint budget.
Reference skills must also set parents and reference together, because
the catalog reads one and materialization and file lookup read the other.

feat(core): Load Instance AI skill references progressively (no-changelog)

A references/*.md file with skill frontmatter is now a reference skill.
The catalog does not list it. Its parent skill lists it, with a
description, when the parent loads. The model loads it by id. A skill
can list references of another skill with shared_references.
Eight workflow sub-step skills are now references of workflow-builder,
which makes the catalog smaller. Active-skill instructions get their own
cached system message, so a skill change keeps the base instructions
cached. The config evaluation flag also hides config-eval-playbook.

executions(get) needs the workflow ID to report ranPublishedVersion. A
running execution now returns its workflow ID and workflow version ID.

feat(core): Keep nested tool field descriptions and shorten Instance AI tool descriptions (no-changelog)

Strict schema sanitizing rebuilt objects, arrays, and optional fields
without their descriptions, so the model did not get them. The sanitizer
now copies the description onto the rebuilt node.
Tool and field descriptions are shorter, to limit the larger payload.
executions(get) now returns ranPublishedVersion, the status filter is an
enum, and run-step shows only when the service supports it.
