refactor(core): Drop the unreachable parent node guard 
The requesting node falls back to its own name, so the parent name is always
set and the guard that reported a missing parent can no longer run. Removing
it also removes the two parameters that only fed the report.

fix(core): Detect paired item cycles inside the traversal memo 
The memo caches an error against the state that produced it. A cycle error
raised for a state that is in flight higher up the walk is only true for that
branch, so caching it made a later, cycle-free branch fail and the result
depend on branch order.

PairedItemMemo now owns the cycle check: it tracks the states in flight and
does not cache a result computed while one of them was in flight. Both keys
were built from the same four fields, so the walk now has one key builder and
no node name can collide with another name plus a run index.
