65e05ec252 s390/vfio-ap: fix KVM GISC and page leak when queue removed from host config
Three related problems exist in the handling of KVM interrupt and page
resources when a queue is removed from the host's AP configuration
while assigned to a mediated device (mdev).

Problem 1:
~~~~~~~~~
AP_RESPONSE_Q_NOT_AVAIL not handled in vfio_ap_mdev_reset_queue()

When the AP bus removes a queue device whose adapter or domain has
been removed from the host's AP configuration,
vfio_ap_mdev_remove_queue() is called. If the queue is still in the
host's AP configuration at that point, it calls
vfio_ap_mdev_reset_queue(), which issues a PQAP(ZAPQ). Since the
adapter is already gone from the host configuration, ap_zapq() returns
AP_RESPONSE_Q_NOT_AVAIL (0x01). This response code is not handled in
vfio_ap_mdev_reset_queue()'s switch statement and falls through to
the default case, which issues a WARN but does not call
vfio_ap_free_aqic_resources(). As a result, if IRQ handling was
enabled for the queue by the guest, the KVM GISC registration and
the pinned guest page holding the notification indicator byte (NIB)
are both leaked.

This is fixed by adding AP_RESPONSE_Q_NOT_AVAIL to the same case as
AP_RESPONSE_DECONFIGURED and AP_RESPONSE_CHECKSTOPPED in
vfio_ap_mdev_reset_queue(). Like those response codes, Q_NOT_AVAIL
indicates the queue is not operational and no further reset attempts
are possible; the correct action is to free the IRQ resources
immediately.

Problem 2:
~~~~~~~~~
 AP_RESPONSE_Q_NOT_AVAIL not handled in apq_status_check()

In vfio_ap_mdev_reset_queue(), there are four cases that indicate a queue
reset has not yet completed, in which case apq_reset_check() is queued to
a work queue to verify completion of the reset operation. This function
uses the PQAP(TAPQ) function to get the queue's status and calls
apq_status_check() to verify whether the reset has completed, failed or
needs to be executed again. As described in Problem #1 above,
apq_reset_check() does not specifically check for AP_RESPONSE_Q_NOT_AVAIL,
thereby potentially leaking KVM GISC registration and the pinned guest page
holding the NIB.

This is fixed by adding a case statement for AP_RESPONSE_Q_NOT_AVAIL to
apq_status_check() and returning -ENODEV for that case. The caller,
apq_reset_check() will then check for this return code and call
vfio_ap_free_aqic_resources() to prevent the leak.

Problem 3:
~~~~~~~~~
vfio_ap_free_aqic_resources() leaks saved_isc when kvm is NULL

vfio_ap_free_aqic_resources() guards the call to
kvm_s390_gisc_unregister() with:

    if (q->saved_isc != VFIO_AP_ISC_INVALID &&
        !WARN_ON(!(q->matrix_mdev && q->matrix_mdev->kvm)))

If matrix_mdev->kvm is NULL -- which can happen when
vfio_ap_mdev_unset_kvm() has already run and cleared kvm before a
subsequent cleanup path reaches this function -- the WARN_ON fires
and the entire block is skipped. This leaves q->saved_isc set to a
non-invalid value, creating a potential double-free on any subsequent
call to this function.

When kvm is NULL the KVM guest is already torn down, so
kvm_s390_gisc_unregister() need not and cannot be called; however,
q->saved_isc must always be cleared. Fix this by separating the
kvm_s390_gisc_unregister() call from the q->saved_isc reset. The
WARN_ON now guards only the genuinely impossible case of matrix_mdev
being NULL. A NULL kvm is handled gracefully by skipping only the
unregister call, and q->saved_isc = VFIO_AP_ISC_INVALID is set
unconditionally whenever saved_isc was not already invalid.

Additionally, add an else clause to the host-config check in
vfio_ap_mdev_remove_queue() to call vfio_ap_free_aqic_resources()
directly when the queue is not in the host's AP configuration. This
serves as a backstop: when the AP bus fires the driver .remove
callback after an adapter is removed from the host config, the queue
is by definition no longer addressable, so vfio_ap_mdev_reset_queue()
would always return Q_NOT_AVAIL. The else clause handles this case
directly without the unnecessary ap_zapq() call, and ensures cleanup
occurs even if kvm has already been set to NULL by a prior call to
vfio_ap_mdev_unset_kvm().

27554b9505 KVM: s390: Fix potential races in dat skey functions
When dat_cond_set_storage_key() finds a large page, it will
conditionally set the storage key in absolute memory using
large_crste_to_phys() to get the absolute address.

There is a race window between dat_entry_walk() and
large_crste_to_phys(): the large page could have been split
concurrently, and large_crste_to_phys() might be called with a crste
that does not designate a large page, leading to crashes.

Similar issues were also present in dat_set_storage_key().

dat_get_storage_key() and dat_reset_reference_bit() did instead check
for a potential concurrent splitting of the large page, but then
handled it incorrectly.

Fix by performing a READ_ONCE on the crste pointer, checking and using
the result, instead of dereferencing the pointer again. In case a race
is detacted, try dat_entry_walk() again.

19192a4043 KVM: s390: Move all code into s390_kvm_mmu_prepare_memory_region()
Move all code from s390_kvm_mmu_commit_memory_region() into
s390_kvm_mmu_prepare_memory_region(). This allows the function to fail
gracefully if needed. The previous behaviour was to print a warning and
continue execution with page tables inconsistent with the memslots.

00c0ae5e43 KVM: s390: Refactor dat_set_slot()
Refactor dat_set_slot(), _dat_slot_pte(), _dat_slot_crste(). Now they
only take a struct kvm_s390_mmu_cache as priv. For dat_delete_slot(),
mc is NULL, as no allocations should take place.

This is needed as a prerequisite to move gmap DAT table setup from
kvm_arch_commit_memory_region() to kvm_arch_prepare_memory_region().

faff4c8ff3 KVM: s390: Fix _gaccess_shadow_fault()
In some circumstances, it is possible that the page of nested guest
memory that is being shadowed is not present at all in the parent guest
gmap. dat_entry_walk() will not find any leaf entry and return with
-ENOENT, which will erroneously be propagated all the way to userspace.

Fix by manually calling gmap_link() on the memory of the nested guest
that is being shadowed if the mapping was not already present.

d215f014b3 KVM: s390: Fix dirty marking in adapter_indicators_set*()
When the indicator and/or summary bits are set in the guest, the
accessed page was only marked dirty in KVM if the access was performed
using the slow path; accesses through the new kvm_arch_set_irq_inatomic
fast inject path would not mark the page as dirty.

Fix by adding/moving the missing calls to mark_page_dirty(). Note that
for the inatomic path set_page_dirty{,_lock}() is not needed as the
page stays pinned; the unpin path correctly marks it as dirty.

Opportunistically reorder the local variables to be in reverse
Christmas tree order and refactor to use guard().

93e257938a tools/workqueue/wq_dump.py: Support backward compatibility for wq->attrs rename
Commit 464e454e1c ("workqueue: rename wq->unbound_attrs to wq->attrs")
renamed wq->unbound_attrs to wq->attrs. When running wq_dump.py against
older running kernels or vmcores where struct workqueue_struct still
contains unbound_attrs, drgn raises an AttributeError.

Add a wq_attrs() helper to allow wq_dump.py to inspect both older and newer
kernel versions seamlessly.

bf1079577a sched_ext: Fixes for v7.3-rc1
- The task ownership check in the dispatch queue move operation raced
   against the task exiting or moving to a different sub-scheduler,
   spuriously triggering scheduler aborts. Fix by moving the check under
   the queue lock.
 
 - The cgroup bandwidth change callback runs in a sleepable context but
   sleepable implementations were rejected at load time. Allow them and add
   a marker so userspace can detect the capability.

068c35b5d0 workqueue: reject watchdog thresholds that overflow jiffies
The watchdog threshold is supplied in seconds but is multiplied by HZ
before being used as a jiffies interval. Reject values that exceed
MAX_JIFFY_OFFSET / HZ so the multiplication cannot wrap and the
time_after() comparisons remain within their supported range.

The check is performed before changing the threshold or watchdog timer.
Zero remains the value used to disable the watchdog.

monitor_timerfn(), lowpri_timerfn() and round_robin_timerfn() ignore
bpf_timer_start()'s return value: a failed re-arm silently stops the
periodic heartbeat, starving every task parked in LOWPRI_DSQ (lowpri)
or freezing cid rotation (round-robin).  Check the returns and raise
scx_bpf_error(), matching the init paths.

b6ee92d7f7 sched_ext: Fix vtime delta loss in scx_flatcg cgroup migration
fcg_cgroup_move() lost the signed vtime offset across cgroup
migration in the mechanical conversion to time helpers:
time_delta() clamps negative deltas to 0, so a queued task (whose
dsq_vtime is normally behind the source frontier) loses its
accumulated vtime credit and lands exactly at the destination
frontier instead of keeping its relative position.  Restore the
wrapping signed subtraction.

2376135986 sched_ext: Fix timer pinning and return value in scx_central
central_timerfn() re-arms the timer with a hardcoded
BPF_F_TIMER_CPU_PIN flag and ignores the return value, defeating
start_central_timer()'s -EINVAL fallback for kernels without the flag
(<6.7): on such kernels the first tick kills the timer permanently
with no diagnostic.  Honor timer_pinned and check the return like
the initial arm does.

3f4b7d1a49 selftests/cgroup: test clone3() into a previously killed cgroup
Once cgroup.kill had been written to a cgroup, a stale kill_seq
snapshot (taken in cgroup_css_set_fork() before the target cgroup was
resolved) caused every child subsequently cloned into that cgroup with
clone3(CLONE_INTO_CGROUP) to be SIGKILLed on the spot.

Add a regression test: create a cgroup, kill it while it is empty,
then clone a child into it and check that the child runs and exits
cleanly. On a kernel without the fix, the test fails:

  not ok 4 test_cgkill_clone_into_killed

The test is skipped on kernels without clone3() or without
CLONE_INTO_CGROUP.

8e35992021 cgroup: fix spurious SIGKILL of CLONE_INTO_CGROUP children
Since commit b69bb476de ("cgroup: fix race between fork and
cgroup.kill"), the fork path snapshots the kill_seq of the child's
future cgroup into kargs->kill_seq, and cgroup_post_fork() SIGKILLs
the child if that cgroup's kill_seq has changed in the meantime, to
catch forks racing with a cgroup.kill sweep.

For CLONE_INTO_CGROUP, however, the snapshot in cgroup_css_set_fork()
is taken before the target cgroup has been resolved: kargs->cgrp is
always NULL at this point (it is only set at the end of the function).
So the "if (kargs->cgrp)" branch is dead code and the snapshot always
records the kill_seq of the parent's cgroup. cgroup_post_fork() then
compares it with the kill_seq of the target cgroup, so the child gets
SIGKILLed whenever the two cgroups have been killed a different number
of times.

As a result, once cgroup.kill has been written to a cgroup, every
child subsequently cloned into it with clone3(CLONE_INTO_CGROUP) is
killed on the spot, for as long as the cgroup exists: kill_seq is not
exposed to userspace and never resets.

Re-snapshot kill_seq from the target cgroup once it has been resolved,
and drop the dead branch at the early snapshot site.

This does not reopen the race fixed by b69bb476de. For
CLONE_INTO_CGROUP, everything from the snapshot to the check in
cgroup_post_fork() runs with cgroup_mutex held, and kill_seq is
only ever incremented under cgroup_mutex.

   Currently if a boot instance creation fails, instead of printing out the
   name of the instance that failed, it prints "(null)". That is because it
   prints "cur_str" that had already been processed by strsep(). Print the
   saved name instead.
 
   While at it, print the error code of the failure.
 
 - Fix use-after-free for same named historgrams
 
   Histograms can be named so that they can be used in multiple events. But
   if the named histogram has a variable attached, the second event that uses
   the named histogram which duplicates it and needs to free the original
   after duplication leaves the old variable in place and still visible. If
   another histogram uses than variable, it will use the stale one which will
   try to reference the freed duplicate histogram and crash the kernel.
 
   Free the duplicate variables along with the duplicated histogram data.
 
 - Check return value of kthread_run() in event self test
 
   The events self tests uses a kthread for testing but does not check if it
   succeeded in creating a kthread. If the kthread creation were to fail, the
   code will still try to call kthread_stop() on the error returned.

   of the "events" directory, the children lists have not yet been
   initialized and the free will trigger a warning because list_empty() on an

   Move the initialization into init_ei() where it makes more sense and makes
   sure that a created eventfs_inode has its lists initialized upon creation.
 
 - Check return value of kthread_run() in ftrace direct sample code
 
   The sample code that shows how to use the ftrace direct calls does not
   test the return of kthread_run() to see if it succeeds. Return a failure
   if the kthread_run() doesn't succeed.
 
 - Clear user events state on fork in case of alloc failure
 
   On fork, the child gets a pointer to the parent's user events state. It
   makes a copy of it then updates the child's pointer to it. But if the
   allocation fails, the duplication function leaves the child with a pointer
   to its parent's descriptor. When the child cleans up its data, it will free
   the parent's descriptor while the parent is still using it.
 
   In the duplication function, set the child's user_event_mm to NULL before
   testing if the allocation succeeded, and when it exits it will not free
   the parent's descriptor.
 
 - Fix retry exhaustion in simple ring buffer reader swap
 
   simple_ring_buffer_swap_reader_page() starts with retry set to 8 and
   post-decrements it only after a failed link replacement. On the final
   attempt, a successful replacement leaves retry at zero, while a failed

78004e9a87 openrisc: fix arbitrary kernel memory access via or1k_atomic syscall
sys_or1k_atomic() (syscall 244 in the "or1k" ABI) takes two user
pointers, v1 and v2, and swaps the words they point to in hand-written
assembly.

    l.lwz   r29,0(r4)

    l.sw    0(r5),r29

The pointers are not checked with access_ok(). The four memory
accesses also have no exception table entries.

A caller passes a kernel address as either pointer, and the syscall
reads from and writes to it directly.

This gives an unprivileged process a kernel read/write primitive. It
overwrites kernel data such as the sys_call_table, gaining code
execution in kernel context.

Check both pointers before entering the critical section. Add fixups
for the four memory accesses so faults on valid but unmapped user
addresses return -EFAULT.
