Introduce a vfio driver over virtio devices to support the legacy
interface functionality for VFs.
Background, from the virtio spec [1].

In some systems, there is a need to support a virtio legacy driver with
a device that does not directly support the legacy interface. In such
scenarios, a group owner device can provide the legacy interface
functionality for the group member devices. The driver of the owner
device can then access the legacy interface of a member device on behalf
of the legacy member device driver.
For example, with the SR-IOV group type, group members (VFs) can not
present the legacy interface in an I/O BAR in BAR0 as expected by the
legacy pci driver. If the legacy driver is running inside a virtual
machine, the hypervisor executing the virtual machine can present a
virtual device with an I/O BAR in BAR0. The hypervisor intercepts the
legacy driver accesses to this I/O BAR and forwards them to the group
owner device (PF) using group administration commands.

Specifically, this driver adds support for a virtio-net VF to be exposed
as a transitional device to a guest driver and allows the legacy IO BAR
functionality on top.
This allows a VM which uses a legacy virtio-net driver in the guest to
work transparently over a VF which its driver in the host is that new
driver.
The driver can be extended easily to support some other types of virtio
devices (e.g virtio-blk), by adding in a few places the specific type
properties as was done for virtio-net.
For now, only the virtio-net use case was tested and as such we introduce
the support only for such a device.
Practically,
Upon probing a VF for a virtio-net device, in case its PF supports
legacy access over the virtio admin commands and the VF doesn't have BAR
0, we set some specific 'vfio_device_ops' to be able to simulate in SW a
transitional device with I/O BAR in BAR 0.
The existence of the simulated I/O bar is reported later on by
overwriting the VFIO_DEVICE_GET_REGION_INFO command and the device
exposes itself as a transitional device by overwriting some properties
upon reading its config space.
Once we report the existence of I/O BAR as BAR 0 a legacy driver in the
guest may use it via read/write calls according to the virtio
specification.
Any read/write towards the control parts of the BAR will be captured by
the new driver and will be translated into admin commands towards the
device.
In addition, any data path read/write access (i.e. virtio driver
notifications) will be captured by the driver and forwarded to the
physical BAR which its properties were supplied by the admin command
VIRTIO_ADMIN_CMD_LEGACY_NOTIFY_INFO upon the probing/init flow.
With that code in place a legacy driver in the guest has the look and
feel as if having a transitional device with legacy support for both its
control and data path flows.

Expose vfio_pci_core_iowrite/read##size() to let it be used by drivers.
This functionality is needed to enable direct access to some physical
BAR of the device with the proper locks/checks in place.
The next patches from this series will use this functionality on a data
path flow when a direct access to the BAR is needed.

Expose vfio_pci_core_setup_barmap() to be used by drivers.
This will let drivers to mmap a BAR and re-use it from both vfio and the
driver when it's applicable.
This API will be used in the next patches by the vfio/virtio coming

Introduce APIs to execute legacy IO admin commands.
It includes: io_legacy_read/write for both common and the device
configuration, io_legacy_notify_info.
In addition, exposing an API to check whether the legacy IO commands are
supported. (i.e. virtio_pci_admin_has_legacy_io()).
Those APIs will be used by the next patches from this series.
Note:
Unlike modern drivers which support hardware virtio devices, legacy
drivers assume software-based devices: e.g. they don't use proper memory
barriers on ARM, use big endian on PPC, etc. X86 drivers are mostly ok
though, more or less by chance. For now, only support legacy IO on X86.

Add support for sending admin command through admin virtqueue interface.
Abort any inflight admin commands once device reset completes. Activate
admin queue when device becomes ready; deactivate on device reset.
To comply to the below specification statement [1], the admin virtqueue
is activated for upper layer users only after setting DRIVER_OK status.
[1] The driver MUST NOT send any buffer available notifications to the
device before setting DRIVER_OK.

Introduce support for the admin virtqueue. By negotiating
VIRTIO_F_ADMIN_VQ feature, driver detects capability and creates one
administration virtqueue. Administration virtqueue implementation in
virtio pci generic layer, enables multiple types of upper layer
drivers such as vfio, net, blk to utilize it.

Only supporting a single region/range is limiting,
wasteful, and in some cases broken (i.e. when there
are large gaps in the iova memory ranges). Fix this

Since the seq/ack operations never happen in parallel there
is no need for multiple scatter gather lists per region.
The current implementation is wasting memory. Fix this by
only using a single scatter-gather list for both the seq
and ack operations.

The incorrect check is being done for comparing the
iova/length being requested to sync. This can cause
the dirty sync operation to fail. Fix this by making
sure the iova offset added to the requested sync
length doesn't exceed the region_size.
Also, the region_start is assumed to always be at 0.
This can cause dirty tracking to fail because the
device/driver bitmap offset always starts at 0,
however, the region_start/iova may not. Fix this by
determining the iova offset from region_start to
determine the bitmap offset.

There are multiple devices, software and operational steps involved
in the process of live migration. An error occurred on any node may
cause the live migration operation to fail.
This complex process makes it very difficult to locate and analyze
the cause when the function fails.
In order to quickly locate the cause of the problem when the
live migration fails, I added a set of debugfs to the vfio
live migration driver.

The block layer doesn't support logical block sizes smaller than 512
bytes. The nvme spec doesn't support that small either, but the driver
isn't checking to make sure the device responded with usable data.
Failing to catch this will result in a kernel bug, either from a
division by zero when stacking, or a zero length bio.

Request queue quiesce may interrupt flush sequence, and the original request
may have been marked as COMPLETE, but can't get finished because of
queue quiesce.
This way is fine from driver viewpoint, because flush sequence is block
layer concept, and it isn't related with driver.
However, driver(such as dm-rq) can call blk_mq_queue_inflight() to count &
drain inflight requests, then the wait & drain never gets done because
the completed & not-finished flush request is counted as inflight.
Fix this issue by not counting completed flush data request as inflight in
case of quiesce.

Since the rethook::handler is an RCU-maganged pointer so that it will
notice readers the rethook is stopped (unregistered) or not, it should
be an __rcu pointer and use appropriate functions to be accessed. This
will use appropriate memory barrier when accessing it. OTOH,
rethook::data is never changed, so we don't need to check it in
get_kretprobe().
NOTE: To avoid sparse warning, rethook::handler is defined by a raw
function pointer type with __rcu instead of rethook_handler_t.

It seems that the pointer-to-kretprobe "rp" within the kretprobe_holder is
RCU-managed, based on the (non-rethook) implementation of get_kretprobe().
The thought behind this patch is to make use of the RCU API where possible
when accessing this pointer so that the needed barriers are always in place
and to self-document the code.
The __rcu annotation to "rp" allows for sparse RCU checking. Plain writes
done to the "rp" pointer are changed to make use of the RCU macro for
assignment. For the single read, the implementation of get_kretprobe()
is simplified by making use of an RCU macro which accomplishes the same,
but note that the log warning text will be more generic.
I did find that there is a difference in assembly generated between the
usage of the RCU macros vs without. For example, on arm64, when using
rcu_assign_pointer(), the corresponding store instruction is a
store-release (STLR) which has an implicit barrier. When normal assignment
is done, a regular store (STR) is found. In the macro case, this seems to
be a result of rcu_assign_pointer() using smp_store_release() when the
value to write is not NULL.

objpool overrun stress with test_objpool on OrangePi5+ SBC triggered the
following kernel warnings:

The overrun test case is to validate the case that pre-allocated objects
are insufficient: 8 objects are pre-allocated for each node and consumer
thread per node tries to grab 16 objects in a row. The testing system is
OrangePI 5+, with RK3588, a big.LITTLE SOC with 4x A76 and 4x A55. When
disabling either all 4 big or 4 little cores, the overrun tests run well,
and once with big and little cores mixed together, the overrun test would
always cause an overrun loop. It's likely the memory timing differences
of big and little cores cause this trouble. Here are the debugging data
of objpool_try_get_slot after try_cmpxchg_release:

The local copies of 'head' and 'last' were 278 and 276, and reloading of

'slot->head' became 'head + 1', which is correct. But what's wrong here
is the stale value of 'last', and that stale value of 'last' finally led
the overrun of 'head'.
Memory updating of 'last' and 'head' are performed in push() and pop()
independently, which could be the culprit leading this out of order
visibility of 'last' and 'head'. So for objpool_try_get_slot(), it's
not enough only checking the condition of 'head != slot', the implicit
condition 'last - head <= nr_objs' must also be explicitly asserted to
guarantee 'last' is always behind 'head' before the object retrieving.
This patch will check and try reloading of 'head' and 'last' to ensure
'last' is behind 'head' at the time of object retrieving. Performance
testings show the average impact is about 0.1% for X86_64 and 1.12% for
ARM64. Here are the results:
