fix(core): Include indexes and TOAST in Postgres data table size measurement 
Switch the Postgres size query from pg_relation_size to pg_total_relation_size
so TOAST relations and indexes count toward the data table size limit. Tighten
the size integration tests with raw-payload lower bounds and add a differential
test that catches large payloads being uncounted.
