userfaultfd: wait on source PMD during UFFDIO_MOVE
move_pages_huge_pmd() snapshots src_pmdval under src_ptl, drops the lock,
and, for migration entries, waits with pmd_migration_entry_wait().
Passing &src_pmdval is wrong. pmd_migration_entry_wait() must lock and
re-read the real page-table PMD; on split-PMD-lock kernels, a stack
address also resolves to the wrong lock. softleaf_entry_wait_on_locked()
then waits without a folio reference, which is safe only while serialized
against migration-entry removal by the real PT lock.
Pass src_pmd, matching __handle_mm_fault() and hmm_vma_walk_pmd().

mm/huge_memory: set PG_has_hwpoisoned only after new folio head is established
__split_folio_to_order() copies the hwpoison state onto each new sub-folio while splitting a folio to a non-zero order. It does so via if (handle_hwpoison && page_range_has_hwpoisoned(new_head, new_nr_pages)) folio_set_has_hwpoisoned(new_folio); *before* clear_compound_head(new_head)/prep_compound_page(new_head, ...) turns @new_head from a tail page into a proper folio head. PG_has_hwpoisoned is a FOLIO_SECOND_PAGE flag, so folio_set_has_hwpoisoned() resolves to folio_flags(folio, 1). With the new compound_info-based page-flags layout, folio_flags() asserts the page is not a tail: VM_BUG_ON_PGFLAGS(page->compound_info & 1, page); VM_BUG_ON_PGFLAGS(n > 0 && !test_bit(PG_head, &page->flags.f), page); At the current call site @new_head still has the tail marker (compound_info bit 0 set, PG_head clear), so on CONFIG_DEBUG_VM kernels this hits: kernel BUG at include/linux/page-flags.h:354 folio_flags+0x82 folio_set_has_hwpoisoned __split_folio_to_order __split_unmapped_folio __folio_split truncate_inode_partial_folio (shmem hole-punch / MADV_REMOVE) Reproduced by syzkaller: hwpoison-inject a few subpages of a large shmem folio, then MADV_REMOVE (fallocate punch hole) on the same range, which splits the partial folio to a non-zero order. memory_failure() tries to split the poisoned folio to order 0 first, but that split is best-effort; when it fails the folio is left large with PG_has_hwpoisoned set, the case |

mm/khugepaged: enable clean pagecache folio collapse for writable files
collapse_file() is capable of collapsing pagecache folios from writable files to PMD folios. Now enable clean pagecache folio collapse in addition to read-only pagecache folio collapse by removing the inode_is_open_for_write() from file_thp_enabled() and only performing filemap_flush() if the file is read-only. This means userspace needs to explicitly flush the content of pagecache folios before khugepaged can collapse the folios, or use madvise(MADV_COLLAPSE), which does the flush in the retry. The reason is that blindly enabling dirty pagecache folio from writable files collapse makes khugepaged flush these folios all the time. It is undesirable to cause system level pagecache flushes. To properly support dirty pagecache folio collapse, filemap_flush() needs to be avoided. Potentially, merging associated buffer instead of dropping it with filemap_release_folio() might be needed. NOTE: this breaks khugepaged selftests for writable file pagecache collapse, which is set to fail all the time. The next commit fixes it. mm: fs: remove filemap_nr_thps*() functions and their users
They are used by READ_ONLY_THP_FOR_FS to handle writes to FSes without large folio support, so that read-only THPs created in these FSes are not seen by the FSes when the underlying fd becomes writable. Now read-only PMD THPs only appear in a FS with large folio support and the supported orders include PMD_ORDER. READ_ONLY_THP_FOR_FS was using mapping->nr_thps, inode->i_writecount, and smp_mb() to prevent writes to a read-only THP and collapsing writable folios into a THP. In collapse_file(), mapping->nr_thps is increased, then smp_mb(), and if inode->i_writecount > 0, collapse is stopped, while do_dentry_open() first increases inode->i_writecount, then a full memory fence, and if mapping->nr_thps > 0, all read-only THPs are truncated. Now this mechanism can be removed along with READ_ONLY_THP_FOR_FS code, since a dirty folio check has been added after try_to_unmap() in collapse_file() to prevent dirty folios from being collapsed as clean. Replace it with a check on the max folio order of the file's address space mapping, making sure PMD folio is supported. Keep the inode open-for-write check, since even if collapse_file() now makes sure all to-be-collapsed folios are clean and the created PMD file THP can be handled by FSes properly, the filemap_flush() could perform undesirable write back. mm/khugepaged: add per-order mTHP collapse failure statistics
Add three new mTHP statistics to track collapse failures for different orders when encountering swap PTEs, excessive none PTEs, and shared PTEs: - collapse_exceed_swap_pte: Increment when mTHP collapse fails due to encountering a swap PTE. - collapse_exceed_none_pte: Counts when mTHP collapse fails due to exceeding the none PTE threshold for the given order - collapse_exceed_shared_pte: Counts when mTHP collapse fails due to encountering a shared PTE. These statistics complement the existing THP_SCAN_EXCEED_* events by providing per-order granularity for mTHP collapse attempts. The stats are exposed via sysfs under `/sys/kernel/mm/transparent_hugepage/hugepages-*/stats/` for each supported hugepage size. As we currently do not support collapsing mTHPs that contain a swap or shared entry, those statistics keep track of how often we are encountering failed mTHP collapses due to these restrictions. We will add support for mTHP collapse for anonymous pages next; lets also track when this happens at the PMD level within the per-mTHP stats. DAMOS quota often exhausts prematurely because it charges for all memory
attempted, causing slow and inconsistent performance when actions fail
on unreclaimable memory.
To fix this, a new feature lets users set a smaller, flexible quota
charge ratio (via a numerator and denominator) for failed regions.
Since failed actions cause less overhead, reducing their quota cost
ensures more predictable and efficient DAMOS processing
- The 8 patch series "selftests/cgroup: improve zswap tests robustness
and support large page sizes" from Li Wang fixes various spurious
failures and improves the overall robustness of the cgroup zswap
selftests.

mm/huge_memory: preserve pmd_swp_uffd_wp on device-private PMD downgrade
change_non_present_huge_pmd() rewrites a writable device-private PMD swap
entry into a readable one without carrying pmd_swp_uffd_wp() across. The
PTE-level change_softleaf_pte() does this correctly; mirror that here,
matching what copy_huge_pmd() does for the fork path. Without the carry,
a plain mprotect() over a UFFD_WP-marked device-private THP strips the bit
and the trap is bypassed on swap-in.

mm/thp: clear deferred split shrinker bits when queues drain
deferred_split_count() returns the raw list_lru count. When the per-memcg, per-node list is empty, that count is 0. That skips scanning, but it does not tell memcg reclaim that the shrinker is empty. shrink_slab_memcg() only clears the memcg shrinker bit when the count callback reports SHRINK_EMPTY. Return SHRINK_EMPTY for an empty deferred split list, so the bit can be cleared once the queue has drained. The deferred split queue handles cgroups in a suboptimal fashion. The
queue is per-NUMA node or per-cgroup, not the intersection. That means on
a cgrouped system, a node-restricted allocation entering reclaim can end

memcg, including those on other nodes, will be split.
list_lru properly sets up per-node, per-cgroup lists. As a bonus, it
streamlines a lot of the list operations and reclaim walks. It's used
widely by other major shrinkers already. Convert the deferred split queue
as well.
The list_lru per-memcg heads are instantiated on demand when the first
object of interest is allocated for a cgroup, by calling
folio_memcg_alloc_deferred(). Add calls to where splittable pages are
created: anon faults, swapin faults, khugepaged collapse.
These calls create all possible node heads for the cgroup at once, so the
migration code (between nodes) doesn't need any special care.

mm: preserve PG_dropbehind flag during folio split
__split_folio_to_order() copies page flags from the original folio to
newly created sub-folios using an explicit allowlist, but PG_dropbehind
is not included. When a large folio with PG_dropbehind set is split,
only the head sub-folio retains the flag; all tail sub-folios silently
lose it and will not be reclaimed eagerly after writeback completes.
Add PG_dropbehind to the flag copy mask so that the drop-behind hint
is preserved across folio splits.

The cgroup v1 swap helpers always operate on swap cache folios whose swap entry is stable: the folio is locked and in the swap cache. There is no need to pass the swap entry or page count as separate parameters when they can be derived from the folio itself. Simplify the redundant parameters and add sanity checks to document the required preconditions. Also rename memcg1_swapout to __memcg1_swapout to indicate it requires special calling context: the folio must be isolated and dying, and the call must be made with interrupts disabled. Patch series "mm: huge_memory: clean up defrag sysfs with shared", v2.
Refactor defrag_store() and defrag_show() to use shared data tables
instead of duplicated if/else chains.
Patch 1 introduces an enum defrag_mode, a defrag_mode_strings[] table, and
a defrag_flags[] mapping array, then rewrites defrag_store() to use
sysfs_match_string() with a loop over defrag_flags[].
Patch 2 refactors defrag_show() to use the same arrays, replacing its
hardcoded if/else chain of test_bit() calls and string literals.
This follows the same pattern applied to anon_enabled_store() in commit
522dfb4ba71f ("mm: huge_memory: refactor anon_enabled_store() with
change_anon_orders()").
This patch (of 2):
Replace the if/else chain of sysfs_streq() calls in defrag_store() with
sysfs_match_string() and a defrag_mode_strings[] table.
Introduce enum defrag_mode and defrag_flags[] array mapping each mode to
its corresponding transparent_hugepage_flag. The store function now loops
over defrag_flags[], setting the bit for the selected mode and clearing
the others. When mode is DEFRAG_NEVER (index 4), no index in the
4-element defrag_flags[] matches, so all flags are cleared.
Note that the enum ordering (always, defer, defer+madvise, madvise, never)
differs from the original if/else chain order in defrag_store() (always,
defer+madvise, defer, madvise, never). This is intentional to match the
display order used by defrag_show().
This is a follow-up cleanup to commit 522dfb4ba71f ("mm: huge_memory:
refactor anon_enabled_store() with change_anon_orders()") which applied
the same sysfs_match_string() pattern to anon_enabled_store().

mm: thp: prevent memory cgroup release in folio_split_queue_lock{_irqsave}()
In the near future, a folio will no longer pin its corresponding memory
cgroup. To ensure safety, it will only be appropriate to hold the rcu
read lock or acquire a reference to the memory cgroup returned by
folio_memcg(), thereby preventing it from being released.
In the current patch, the rcu read lock is employed to safeguard against
the release of the memory cgroup in folio_split_queue_lock{_irqsave}().

mm: rename unlock_page_lruvec_irq and its variants
It is inappropriate to use folio_lruvec_lock() variants in conjunction
with unlock_page_lruvec() variants, as this involves the inconsistent
operation of locking a folio while unlocking a page. To rectify this, the
functions unlock_page_lruvec{_irq, _irqrestore} are renamed to
lruvec_unlock{_irq,_irqrestore}.

migrate_folio_move() records the deferred split queue state from src and replays it on dst. Replaying it after remove_migration_ptes(src, dst, 0) makes dst visible before it is requeued, so a concurrent rmap-removal path can mark dst partially mapped and trip the WARN in deferred_split_folio(). Move the requeue before remove_migration_ptes() so dst is back on the deferred split queue before it becomes visible again. Because migration still holds dst locked at that point, teach deferred_split_scan() to requeue a folio when folio_trylock() fails. Otherwise a fully mapped underused folio can be dequeued by the shrinker and silently lost from split_queue. Now we have pmd_to_softleaf_folio() available to us which also raises a CONFIG_DEBUG_VM warning if unexpectedly an invalid softleaf entry, we can now abstract folio handling altogether. vm_normal_folio() deals with the huge zero page (which is present), as well as PFN map/mixed map mappings in both cases returning NULL. Otherwise, we try to obtain the softleaf folio. This makes the logic far easier to comprehend and has it use the standard vm_normal_folio_pmd() path for decoding of present entries. Finally, we have to update the flushing logic to only do so if a folio is established. This patch also makes the 'is_present' value more accurate - because PFN map, mixed map and zero huge pages are present, just not present and 'normal'. These checks have been in place since 2014, I think we can safely assume that we are in a place where we don't need these as runtime checks. In addition there are 4 other invocations of folio_remove_rmap_pmd(), none of which make this assertion. If we need to add this assertion, it should be in folio_remove_rmap_pmd(), and as a VM_WARN_ON_ONCE(), however these seem superfluous so just remove them. mm/huge_memory: add a common exit path to zap_huge_pmd()
Other than when we acquire the PTL, we always need to unlock the PTL, and optionally need to flush on exit. The code is currently very duplicated in this respect, so default flush_needed to false, set it true in the case in which it's required, then share the same logic for all exit paths. This also makes flush_needed make more sense as a function-scope value (we don't need to flush for the PFN map/mixed map, zero huge, error cases for instance). A recent bug I analysed managed to, through a bug in the userfaultfd implementation, reach an invalid point in the zap_huge_pmd() code where the PMD was none of: - A non-DAX, PFN or mixed map. - The huge zero folio - A present PMD entry - A softleaf entry The code at this point calls folio_test_anon() on a known-NULL folio. Having logic like this explicitly NULL dereference in the code is hard to understand, and makes debugging potentially more difficult. Add an else branch to handle this case and WARN(). No functional change intended. Patch series "mm/huge_memory: refactor zap_huge_pmd()", v3. zap_huge_pmd() is overly complicated, clean it up and also add an assert in the case that we encounter a buggy PMD entry that doesn't match expectations. This is motivated by a bug discovered [0] where the PMD entry was none of: * A non-DAX, PFN or mixed map. * The huge zero folio * A present PMD entry * A softleaf entry In zap_huge_pmd(), but due to the bug we manged to reach this code. It is useful to explicitly call this out rather than have an arbitrary NULL pointer dereference happen, which also improves understanding of what's going on. The series goes further to make use of vm_normal_folio_pmd() rather than implementing custom logic for retrieving the folio, and extends softleaf functionality to provide and use an equivalent softleaf function. This patch (of 13): This function is confused - it overloads the term 'special' yet again, checks for DAX but in many cases the code explicitly excludes DAX before invoking the predicate. It also unnecessarily checks for vma->vm_file - this has to be present for a driver to have set VMA_MIXEDMAP_BIT or VMA_PFNMAP_BIT. In fact, a far simpler form of this is to reverse the DAX predicate and return false if DAX is set. This makes sense from the point of view of 'special' as in vm_normal_page(), as DAX actually does potentially have retrievable folios. Also there's no need to have this in mm.h so move it to huge_memory.c. No functional change intended. Refactor enabled_store() to use a new set_global_enabled_mode() helper. Introduce a separate enum global_enabled_mode and global_enabled_mode_strings[], mirroring the anon_enabled_mode pattern from the previous commit. A separate enum is necessary because the global THP setting does not support "inherit", only "always", "madvise", and "never". Reusing anon_enabled_mode would leave a NULL gap in the string array, causing sysfs_match_string() to stop early and fail to match entries after the gap. The helper uses the same loop pattern as set_anon_enabled_mode(), iterating over an array of flag bit positions and using test_and_set_bit()/test_and_clear_bit() to track whether the state actually changed. Consolidate the repeated spin_lock/set_bit/clear_bit pattern in anon_enabled_store() into a new set_anon_enabled_mode() helper that loops over an orders[] array, setting the bit for the selected mode and clearing the others. Introduce enum anon_enabled_mode and anon_enabled_mode_strings[] for the per-order anon THP setting. Use sysfs_match_string() with the anon_enabled_mode_strings[] table to replace the if/else chain of sysfs_streq() calls. The helper uses __test_and_set_bit()/__test_and_clear_bit() to track whether the state actually changed, so start_stop_khugepaged() is only called when needed. When the mode is unchanged, set_recommended_min_free_kbytes() is called directly to preserve the watermark recalculation behavior of the original code. mm/huge_memory: fix use of NULL folio in move_pages_huge_pmd()
move_pages_huge_pmd() handles UFFDIO_MOVE for both normal THPs and huge zero pages. For the huge zero page path, src_folio is explicitly set to NULL, and is used as a sentinel to skip folio operations like lock and rmap. In the huge zero page branch, src_folio is NULL, so folio_mk_pmd(NULL, pgprot) passes NULL through folio_pfn() and page_to_pfn(). With SPARSEMEM_VMEMMAP this silently produces a bogus PFN, installing a PMD pointing to non-existent physical memory. On other memory models it is a NULL dereference. mm/huge_memory: fix a folio_split() race condition with folio_try_get()
During a pagecache folio split, the values in the related xarray should not be changed from the original folio at xarray split time until all after-split folios are well formed and stored in the xarray. Current use of xas_try_split() in __split_unmapped_folio() lets some after-split folios show up at wrong indices in the xarray. When these misplaced after-split folios are unfrozen, before correct folios are stored via __xa_store(), and grabbed by folio_try_get(), they are returned to userspace at wrong file indices, causing data corruption. More detailed explanation is at the bottom. The reproducer is at: https://github.com/dfinity/thp-madv-remove-test It 1. creates a memfd, 2. forks, 3. in the child process, maps the file with large folios (via shmem code path) and reads the mapped file continuously with 16 threads, 4. in the parent process, uses madvise(MADV_REMOVE) to punch poles in the large folio. Data corruption can be observed without the fix. Basically, data from a wrong page->index is returned. Fix it by using the original folio in xas_try_split() calls, so that folio_try_get() can get the right after-split folios after the original folio is unfrozen. Uniform split, split_huge_page*(), is not affected, since it uses xas_split_alloc() and xas_split() only once and stores the original folio in the xarray. mm: thp: deny THP for files on anonymous inodes
file_thp_enabled() incorrectly allows THP for files on anonymous inodes
(e.g. guest_memfd and secretmem). These files are created via
alloc_file_pseudo(), which does not call get_write_access() and leaves
inode->i_writecount at 0. Combined with S_ISREG(inode->i_mode) being
true, they appear as read-only regular files when
CONFIG_READ_ONLY_THP_FOR_FS is enabled, making them eligible for THP
collapse.
Anonymous inodes can never pass the inode_is_open_for_write() check
since their i_writecount is never incremented through the normal VFS
open path. The right thing to do is to exclude them from THP eligibility
altogether, since CONFIG_READ_ONLY_THP_FOR_FS was designed for real
filesystem files (e.g. shared libraries), not for pseudo-filesystem
inodes.
For guest_memfd, this allows khugepaged and MADV_COLLAPSE to create
large folios in the page cache via the collapse path, but the
guest_memfd fault handler does not support large folios. This triggers
WARN_ON_ONCE(folio_test_large(folio)) in kvm_gmem_fault_user_mapping().
For secretmem, collapse_file() tries to copy page contents through the
direct map, but secretmem pages are removed from the direct map. This
can result in a kernel crash:

Secretmem is not affected by the crash on upstream as the memory failure

Convert 'alloc_obj' family to use the new default GFP_KERNEL argument
This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

treewide: Replace kmalloc with kmalloc_obj for non-scalar types
This is the result of running the Coccinelle script from scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to avoid scalar types (which need careful case-by-case checking), and instead replace kmalloc-family calls that allocate struct or union object instances: Single allocations: kmalloc(sizeof(TYPE), ...) are replaced with: kmalloc_obj(TYPE, ...) Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...) are replaced with: kmalloc_objs(TYPE, COUNT, ...) Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...) are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...) migrate: replace RMP_ flags with TTU_ flags
Instead of translating between RMP_ and TTU_ flags, remove the RMP_ flags and just use the TTU_ flag space; there's plenty available. Possibly we should rename these to RMAP_ flags, and maybe even pass them in through rmap_walk_arg, but that can be done later.
