41041562a7 io_uring/zcrx: fully clean area on error in io_import_umem()
When accounting fails, io_import_umem() sets the page array, etc. and
returns an error expecting that the error handling code will take care
of the rest. To make the next patch simpler, only return a fully
initialised areas from the function.

e5361d25e2 io_uring/zcrx: return back two step unregistration
There are reports where io_uring instance removal takes too long and an
ifq reallocation by another zcrx instance fails. Split zcrx destruction
into two steps similarly how it was before, first close the queue early
but maintain zcrx alive, and then when all inflight requests are
completed, drop the main zcrx reference. For extra protection, mark
terminated zcrx instances in xarray and warn if we double put them.

aa35dd6bdd io_uring/bpf_filters: retain COW'ed settings on parse failures
If io_parse_restrictions() fails, it ends up clearing any restrictions
currently set. The intent is only to clear whatever it already applied,
but it ends up clearing everything, including whatever settings may have
been applied in a copy-on-write fashion already. Ensure that those are
retained.

61a11cf481 io_uring: protect remaining lockless ctx->rings accesses with RCU
Commit 9618908026 addressed one case of ctx->rings being potentially
accessed while a resize is happening on the ring, but there are still
a few others that need handling. Add a helper for retrieving the
rings associated with an io_uring context, and add some sanity checking
to that to catch bad uses. ->rings_rcu is always valid, as long as it's
used within RCU read lock. Any use of ->rings_rcu or ->rings inside
either ->uring_lock or ->completion_lock is sane as well.

Do the minimum fix for the current kernel, but set it up such that this
basic infra can be extended for later kernels to make this harder to
mess up in the future.

111a12b422 io_uring/rsrc: reject zero-length fixed buffer import
validate_fixed_range() admits buf_addr at the exact end of the
registered region when len is zero, because the check uses strict
greater-than (buf_end > imu->ubuf + imu->len).  io_import_fixed()
then computes offset == imu->len, which causes the bvec skip logic
to advance past the last bio_vec entry and read bv_offset from
out-of-bounds slab memory.

Return early from io_import_fixed() when len is zero.  A zero-length
import has no data to transfer and should not walk the bvec array
at all.

b948f9d5d3 io_uring/net: fix slab-out-of-bounds read in io_bundle_nbufs()
sqe->len is __u32 but gets stored into sr->len which is int. When
userspace passes sqe->len values exceeding INT_MAX (e.g. 0xFFFFFFFF),
sr->len overflows to a negative value. This negative value propagates
through the bundle recv/send path:

  1. io_recv(): sel.val = sr->len (ssize_t gets -1)
  2. io_recv_buf_select(): arg.max_len = sel->val (size_t gets
     0xFFFFFFFFFFFFFFFF)
  3. io_ring_buffers_peek(): buf->len is not clamped because max_len
     is astronomically large
  4. iov[].iov_len = 0xFFFFFFFF flows into io_bundle_nbufs()
  5. io_bundle_nbufs(): min_t(int, 0xFFFFFFFF, ret) yields -1,
     causing ret to increase instead of decrease, creating an
     infinite loop that reads past the allocated iov[] array

This results in a slab-out-of-bounds read in io_bundle_nbufs() from
the kmalloc-64 slab, as nbufs increments past the allocated iovec
entries.

5170efd9c3 io_uring/fdinfo: fix OOB read in SQE_MIXED wrap check
__io_uring_show_fdinfo() iterates over pending SQEs and, for 128-byte
SQEs on an IORING_SETUP_SQE_MIXED ring, needs to detect when the second
half of the SQE would be past the end of the sq_sqes array. The current
check tests (++sq_head & sq_mask) == 0, but sq_head is only incremented
when a 128-byte SQE is encountered, not on every iteration. The actual
array index is sq_idx = (i + sq_head) & sq_mask, which can be sq_mask
(the last slot) while the wrap check passes.

Fix by checking sq_idx directly. Keep the sq_head increment so the loop
still skips the second half of the 128-byte SQE on the next iteration.

b59efde9e6 io_uring/fdinfo: fix SQE_MIXED SQE displaying
When displaying pending SQEs for a MIXED ring, each 128-byte SQE
increments sq_head to skip the second slot, but the loop counter is not
adjusted. This can cause the loop to read past sq_tail by one entry for
each 128-byte SQE encountered, displaying SQEs that haven't been made
consumable yet by the application.

Match the kernel's own consumption logic in io_init_req() which
decrements what's left when consuming the extra slot.

418eab7a6f io_uring/kbuf: propagate BUF_MORE through early buffer commit path
When io_should_commit() returns true (eg for non-pollable files), buffer
commit happens at buffer selection time and sel->buf_list is set to
NULL. When __io_put_kbufs() generates CQE flags at completion time, it
calls __io_put_kbuf_ring() which finds a NULL buffer_list and hence
cannot determine whether the buffer was consumed or not. This means that
IORING_CQE_F_BUF_MORE is never set for non-pollable input with
incrementally consumed buffers.

Likewise for io_buffers_select(), which always commits upfront and
discards the return value of io_kbuf_commit().

Add REQ_F_BUF_MORE to store the result of io_kbuf_commit() during early
commit. Then __io_put_kbuf_ring() can check this flag and set
IORING_F_BUF_MORE accordingy.

3ecd3e0314 io_uring/kbuf: fix missing BUF_MORE for incremental buffers at EOF
For a zero length transfer, io_kbuf_inc_commit() is called with !len.
Since we never enter the while loop to consume the buffers,
io_kbuf_inc_commit() ends up returning true, consuming the buffer. But
if no data was consumed, by definition it cannot have consumed the
buffer. Return false for that case.

3e97c2582f io_uring/rw: use cached file rather than req->file
In io_rw_init_file(), req->file is cached in file, yet the former is
still being used when checking for O_DIRECT. As this is post setting
the kiocb flags, the compiler has to reload req->file. Just use the
locally cached file instead.

a68ed2df72 io_uring/poll: fix multishot recv missing EOF on wakeup race
When a socket send and shutdown() happen back-to-back, both fire
wake-ups before the receiver's task_work has a chance to run. The first
wake gets poll ownership (poll_refs=1), and the second bumps it to 2.
When io_poll_check_events() runs, it calls io_poll_issue() which does a
recv that reads the data and returns IOU_RETRY. The loop then drains all
accumulated refs (atomic_sub_return(2) -> 0) and exits, even though only
the first event was consumed. Since the shutdown is a persistent state
change, no further wakeups will happen, and the multishot recv can hang
forever.

Check specifically for HUP in the poll loop, and ensure that another
loop is done to check for status if more than a single poll activation
is pending. This ensures we don't lose the shutdown event.

Implement BPF struct ops registration. It's registered off the BPF
path, and can be removed by BPF as well as io_uring. To protect it,
introduce a global lock synchronising registration. ctx->uring_lock can
be nested under it. ctx->bpf_ops is write protected by both locks and
so it's safe to read it under either of them.

890819248a io_uring/bpf-ops: add kfunc helpers
Add two kfuncs that should cover most of the needs:

1. bpf_io_uring_submit_sqes(), which allows to submit io_uring requests.
   It mirrors the normal user space submission path and follows all
   related io_uring_enter(2) rules. i.e. SQEs are taken from the SQ
   according to head/tail values. In case of IORING_SETUP_SQ_REWIND,
   it'll submit first N entries.

2. bpf_io_uring_get_region() returns a pointer to the specified region,
   where io_uring regions are kernel-userspace shared chunks of memory.
   It takes the size as an argument, which should be a load time
   constant. There are 3 types of regions:
   - IOU_REGION_SQ returns the submission queue.
   - IOU_REGION_CQ stores the CQ, SQ/CQ headers and the sqarray. In
     other words, it gives same memory that would normally be mmap'ed
     with IORING_FEAT_SINGLE_MMAP enabled IORING_OFF_SQ_RING.
   - IOU_REGION_MEM represents the memory / parameter region. It can be
     used to store request indirect parameters and for kernel - user
     communication.

It intentionally provides a thin but flexible API and expects BPF
programs to implement CQ/SQ header parsing, CQ walking, etc. That
mirrors how the normal user space works with rings and should help
to minimise kernel / kfunc helpers changes while introducing new generic
io_uring features.

d0e437b76b io_uring/bpf-ops: implement loop_step with BPF struct_ops
Introduce io_uring BPF struct ops implementing the loop_step callback,
which will allow BPF to overwrite the default io_uring event loop logic.

The callback takes an io_uring context, the main role of which is to be
passed to io_uring kfuncs. The other argument is a struct iou_loop_params,
which BPF can use to request CQ waiting and communicate other parameters.
See the event loop description in the previous patch for more details.

033af2b3eb io_uring: introduce callback driven main loop
The io_uring_enter() has a fixed order of execution: it submits
requests, waits for completions, and returns to the user. Allow to
optionally replace it with a custom loop driven by a callback called
loop_step. The basic requirements to the callback is that it should be
able to submit requests, wait for completions, parse them and repeat.
Most of the communication including parameter passing can be implemented
via shared memory.

The callback should return IOU_LOOP_CONTINUE to continue execution or
IOU_LOOP_STOP to return to the user space. Note that the kernel may
decide to prematurely terminate it as well, e.g. in case the process was
signalled or killed.

The hook takes a structure with parameters. It can be used to ask the
kernel to wait for CQEs by setting cq_wait_idx to the CQE index it wants
to wait for. Spurious wake ups are possible and even likely, the callback
is expected to handle it. There will be more parameters in the future
like timeout.

It can be used with kernel callbacks, for example, as a slow path
deprecation mechanism overwiting SQEs and emulating the wanted
behaviour, however it's more useful together with BPF programs
implemented in following patches.

Note that keeping it separately from the normal io_uring wait loop
makes things much simpler and cleaner. It keeps it in one place instead
of spreading a bunch of checks in different places including disabling
the submission path. It holds the lock by default, which is a better fit
for BPF synchronisation and the loop execution model. It nicely avoids
existing quirks like forced wake ups on timeout request completion. And
it should be easier to implement new features.

Currently, creating an io_uring with IORING_SETUP_IOPOLL requires all
requests issued to it to support iopoll. This prevents, for example,
using ublk zero-copy together with IORING_SETUP_IOPOLL, as ublk
zero-copy buffer registrations are performed using a uring_cmd. There's
no technical reason why these non-iopoll uring_cmds can't be supported.
They will either complete synchronously or via an external mechanism
that calls io_uring_cmd_done(), io_uring_cmd_post_mshot_cqe32(), or
io_uring_mshot_cmd_post_cqe(), so they don't need to be polled.

Allow uring_cmd requests to be issued to IORING_SETUP_IOPOLL io_urings
even if their files don't implement ->uring_cmd_iopoll(). For these
uring_cmd requests, skip initializing struct io_kiocb's iopoll fields,
don't set REQ_F_IOPOLL, and don't set IO_URING_F_IOPOLL in issue_flags.

3a5e96d47f io_uring: count CQEs in io_iopoll_check()
A subsequent commit will allow uring_cmds that don't use iopoll on
IORING_SETUP_IOPOLL io_urings. As a result, CQEs can be posted without
setting the iopoll_completed flag for a request in iopoll_list or going
through task work. For example, a UBLK_U_IO_FETCH_IO_CMDS command could
call io_uring_mshot_cmd_post_cqe() to directly post a CQE. The
io_iopoll_check() loop currently only counts completions posted in
io_do_iopoll() when determining whether the min_events threshold has
been met. It also exits early if there are any existing CQEs before
polling, or if any CQEs are posted while running task work. CQEs posted
via io_uring_mshot_cmd_post_cqe() or other mechanisms won't be counted
against min_events.

Explicitly check the available CQEs in each io_iopoll_check() loop
iteration to account for CQEs posted in any fashion.

A subsequent commit will allow uring_cmds to files that don't implement
->uring_cmd_iopoll() to be issued to IORING_SETUP_IOPOLL io_urings. This
means the ctx's IORING_SETUP_IOPOLL flag isn't sufficient to determine
whether a given request needs to be iopolled.

Introduce a request flag REQ_F_IOPOLL set in ->issue() if a request
needs to be iopolled to completion. Set the flag in io_rw_init_file()
and io_uring_cmd() for requests issued to IORING_SETUP_IOPOLL ctxs. Use
the request flag instead of IORING_SETUP_IOPOLL in places dealing with a
specific request.

A future possibility would be to add an option to enable/disable iopoll
in the io_uring SQE instead of determining it from IORING_SETUP_IOPOLL.

c2c185be5c io_uring/kbuf: check if target buffer list is still legacy on recycle
There's a gap between when the buffer was grabbed and when it
potentially gets recycled, where if the list is empty, someone could've
upgraded it to a ring provided type. This can happen if the request
is forced via io-wq. The legacy recycling is missing checking if the
buffer_list still exists, and if it's of the correct type. Add those
checks.

6f02c6b196 io_uring: fix physical SQE bounds check for SQE_MIXED 128-byte ops
When IORING_SETUP_SQE_MIXED is used without IORING_SETUP_NO_SQARRAY,
the boundary check for 128-byte SQE operations in io_init_req()
validated the logical SQ head position rather than the physical SQE
index.

The existing check:

  !(ctx->cached_sq_head & (ctx->sq_entries - 1))

ensures the logical position isn't at the end of the ring, which is
correct for NO_SQARRAY rings where physical == logical. However, when
sq_array is present, an unprivileged user can remap any logical
position to an arbitrary physical index via sq_array. Setting
sq_array[N] = sq_entries - 1 places a 128-byte operation at the last
physical SQE slot, causing the 128-byte memcpy in
io_uring_cmd_sqe_copy() to read 64 bytes past the end of the SQE
array.

Replace the cached_sq_head alignment check with a direct validation
of the physical SQE index, which correctly handles both sq_array and
NO_SQARRAY cases.

9618908026 io_uring: ensure ctx->rings is stable for task work flags manipulation
If DEFER_TASKRUN | SETUP_TASKRUN is used and task work is added while
the ring is being resized, it's possible for the OR'ing of
IORING_SQ_TASKRUN to happen in the small window of swapping into the
new rings and the old rings being freed.

Prevent this by adding a 2nd ->rings pointer, ->rings_rcu, which is
protected by RCU. The task work flags manipulation is inside RCU
already, and if the resize ring freeing is done post an RCU synchronize,
then there's no need to add locking to the fast path of task work
additions.

Note: this is only done for DEFER_TASKRUN, as that's the only setup mode
that supports ring resizing. If this ever changes, then they too need to
use the io_ctx_mark_taskrun() helper.

People previously asked for the notification CQE to have a different
user_data value from the main request completion. It's useful to
separate buffer and request handling logic and avoid separately
refcounting the request.

Let the user pass the notification user_data in sqe->addr3. If zero,
it'll inherit sqe->user_data as before. It doesn't change the rules for
when the user can expect a notification CQE, and it should still check
the IORING_CQE_F_MORE flag.

One the things the user has always keep in mind is that any user
pointers they put into an SQE is not going to be read by the kernel
until submission happens, and the user has to ensure the pointee stays
alive until then. For example, snippet below will lead to UAF of the on
stack variable ts. Instead of passing the timeout value as a pointer
allow to store it immediately in the SQE. The user has to set a new flag
called IORING_TIMEOUT_IMMEDIATE_ARG, in which case sqe->addr for timeout
or sqe->addr2 for timeout update requests will be interpreted as a time
value in nanosecods.

io_uring_cmd_sock() originally supported two ioctl-based cmd_op
operations. Over time, additional operations were added with tail calls
to their helpers.

This approach resulted in the new operations sharing an ioctl check
with the original operations.

io_uring_cmd_sock() now supports 6 operations, so let's move the
implementation of the original two into their own helper, reducing
io_uring_cmd_sock() to a simple dispatcher.

3d17d76d1f io_uring/zcrx: don't set rx_page_size when not requested
The rx_buf_len parameter was recently added to the Rx zero-copy
implementation. The expectation is that when not set system will
maintain previous behavior and use the default buffer size (PAGE_SIZE).

This works correctly at the iouring level, but we don't preserve
the same "zero means default" semantics when registering the memory
provider on the netdev. mp_param.rx_page_size is unconditionally
set to PAGE_SIZE. This causes __net_mp_open_rxq() to check for
QCFG_RX_PAGE_SIZE support in the driver, and return -EOPNOTSUPP
for drivers that don't advertise it -- even though the user never
asked for large buffers.

Only set mp_param.rx_page_size when rx_buf_len was explicitly provided,

the resulting diff is also then easier to check automatically.

For the same reason the 'flex' versions will be done as a separate
conversion.

scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)

The io_zcrx_put_niov_uref() function uses a non-atomic
check-then-decrement pattern (atomic_read followed by separate
atomic_dec) to manipulate user_refs. This is serialized against other
callers by rq_lock, but io_zcrx_scrub() modifies the same counter with
atomic_xchg() WITHOUT holding rq_lock.

On SMP systems, the following race exists:

The same niov is pushed to the freelist twice, causing free_count to
exceed nr_iovs. Subsequent freelist pushes then perform an out-of-bounds
write (a u32 value) past the kvmalloc'd freelist array into the adjacent
slab object.

Fix this by replacing the non-atomic read-then-dec in
io_zcrx_put_niov_uref() with an atomic_try_cmpxchg loop that atomically
tests and decrements user_refs. This makes the operation safe against
concurrent atomic_xchg from scrub without requiring scrub to acquire
rq_lock.

be3573124e io_uring/bpf_filter: pass in expected filter payload size
It's quite possible that opcodes that have payloads attached to them,
like IORING_OP_OPENAT/OPENAT2 or IORING_OP_SOCKET, that these paylods
can change over time. For example, on the openat/openat2 side, the
struct open_how argument is extensible, and could be extended in the
future to allow further arguments to be passed in.

Allow registration of a cBPF filter to give the size of the filter as
seen by userspace. If that filter is for an opcode that takes extra
payload data, allow it if the application payload expectation is the
same size than the kernels. If that is the case, the kernel supports
filtering on the payload that the application expects. If the size
differs, the behavior depends on the IO_URING_BPF_FILTER_SZ_STRICT flag:

1) If IO_URING_BPF_FILTER_SZ_STRICT is set and the size expectation
   differs, fail the attempt to load the filter.

2) If IO_URING_BPF_FILTER_SZ_STRICT isn't set, allow the filter if
   the userspace pdu size is smaller than what the kernel offers.

3) Regardless if IO_URING_BPF_FILTER_SZ_STRICT, fail loading the filter
   if the userspace pdu size is bigger than what the kernel supports.

An attempt to load a filter due to sizing will error with -EMSGSIZE.
For that error, the registration struct will have filter->pdu_size
populated with the pdu size that the kernel uses.

has reported this, but it does result in -ENOENT potentially being
returned when trying to match on both, rather than actually doing what

600b665b90 io_uring/cmd_net: fix too strict requirement on ioctl
Attempting SOCKET_URING_OP_SETSOCKOPT on an AF_NETLINK socket resulted
in an -EOPNOTSUPP, as AF_NETLINK doesn't have an ioctl in its struct
proto, but only in struct proto_ops.

Prior to the blamed commit, io_uring_cmd_sock() only had two cmd_op
operations, both requiring ioctl, thus the check was warranted.

Since then, 4 new cmd_op operations have been added, none of which
depend on ioctl. This patch moves the ioctl check, so it only applies
to the original operations.

AFAICT, the ioctl requirement was unintentional, and it wasn't
visible in the blamed patch within 3 lines of context.

f4d0668b38 io_uring/openclose: fix io_pipe_fixed() slot tracking for specific slots
__io_fixed_fd_install() returns 0 on success for non-alloc mode
(specific slot), not the slot index. io_pipe_fixed() used this return
value directly as the slot index in fds[], which can cause the reported
values returned via copy_to_user() to be incorrect, or the error path
operating on the incorrect direct descriptor.

Fix by computing the actual 0-based slot index (slot - 1) for specific
slot mode, while preserving the existing behavior for auto-alloc mode
where __io_fixed_fd_install() already returns the allocated index.

a6bded921e io_uring/filetable: clamp alloc_hint to the configured alloc range
Explicit fixed file install/remove operations on slots outside the
configured alloc range can corrupt alloc_hint via io_file_bitmap_set()
and io_file_bitmap_clear(), which unconditionally update alloc_hint to
the bit position. This causes subsequent auto-allocations to fall
outside the configured range.

For example, if the alloc range is [10, 20) and a file is removed at
slot 2, alloc_hint gets set to 2. The next auto-alloc then starts
searching from slot 2, potentially returning a slot below the range.

Fix this by clamping alloc_hint to [file_alloc_start, file_alloc_end)
at the top of io_file_bitmap_get() before starting the search.

0efc331d78 io_uring/rsrc: replace reg buffer bit field with flags
I'll need a flag in the registered buffer struct for dmabuf work, and
it'll be more convenient to have a flags field rather than bit fields,
especially for io_mapped_ubuf initialisation.

We might want to add more flags in the future as well. For example, it
might be useful for debugging and potentially optimisations to split out
a flag indicating the shape of the buffer to gate iov_iter_advance()
walks vs bit/mask arithmetics. It can also be combined with the
direction mask field.

  This first adds support for cBPF filters for opcodes, which enables
  tighter control over what exactly a specific opcode may do. As
  examples, specific support is added for IORING_OP_OPENAT/OPENAT2,
  allowing filtering on resolve flags. And another example is added for
  IORING_OP_SOCKET, allowing filtering on domain/type/protocol. These
  are both common use cases. cBPF was chosen rather than eBPF, because
  the latter is often restricted in containers as well.

  These filters are run post the init phase of the request, which allows
  filters to even dip into data that is being passed in struct in user
  memory, as the init side of requests make that data stable by bringing
  it into the kernel. This allows filtering without needing to copy this
  data twice, or have filters etc know about the exact layout of the
  user data. The filters get the already copied and sanitized data
  passed.

  On top of that support is added for per-task filters, meaning that any
  ring created with a task that has a per-task filter will get those
  filters applied when it's created. These filters are inherited across
  fork as well. Once a filter has been registered, any further added
  filters may only further restrict what operations are permitted.

Currently io_uring supports restricting operations on a per-ring basis.
To use those, the ring must be setup in a disabled state by setting
IORING_SETUP_R_DISABLED. Then restrictions can be set for the ring, and
the ring can then be enabled.

This commit adds support for IORING_REGISTER_RESTRICTIONS with ring_fd
== -1, like the other "blind" register opcodes which work on the task
rather than a specific ring. This allows registration of the same kind
of restrictions as can been done on a specific ring, but with the task
itself. Once done, any ring created will inherit these restrictions.

If a restriction filter is registered with a task, then it's inherited
on fork for its children. Children may only further restrict operations,
not extend them.

Inheriting restrictions include both the classic
IORING_REGISTER_RESTRICTIONS based restrictions, as well as the BPF
filters that have been registered with the task via
IORING_REGISTER_BPF_FILTER.

442ae40660 io_uring/kbuf: fix memory leak if io_buffer_add_list fails
io_register_pbuf_ring() ignores the return value of io_buffer_add_list(),
which can fail if xa_store() returns an error (e.g., -ENOMEM). When this
happens, the function returns 0 (success) to the caller, but the
io_buffer_list structure is neither added to the xarray nor freed.

In practice this requires failure injection to hit, hence not a real
issue. But it should get fixed up none the less.

Add cond_resched() in those dump loops, just in case a lot of entries
are being dumped. And detect invalid CQ ring head/tail entries, to avoid
iterating more than what is necessary. Generally not an issue, but can be
if things like KASAN or other debugging metrics are enabled.

9121466148 io_uring: allow io-wq workers to exit when unused
io_uring keeps a per-task io-wq around, even when the task no longer has
any io_uring instances.

If the task previously used io_uring for file I/O, this can leave an
unrelated iou-wrk-* worker thread behind after the last io_uring
instance is gone.

When the last io_uring ctx is removed from the task context, mark the
io-wq exit-on-idle so workers can go away. Clear the flag on subsequent
io_uring usage.

38aa434ab9 io_uring/io-wq: add exit-on-idle state
io-wq uses an idle timeout to shrink the pool, but keeps the last worker
around indefinitely to avoid churn.

For tasks that used io_uring for file I/O and then stop using io_uring,
this can leave an iou-wrk-* thread behind even after all io_uring
instances are gone. This is unnecessary overhead and also gets in the
way of process checkpoint/restore.

Add an exit-on-idle state that makes all io-wq workers exit as soon as
they become idle, and provide io_wq_set_exit_on_idle() to toggle it.

806ae939c4 io_uring/net: don't continue send bundle if poll was required for retry
If a send bundle has picked a bunch of buffers, then it needs to send
all of those to be complete. This may require poll arming, if the send
buffer ends up being full. Once a send bundle has been poll armed, no
further bundles should be attempted.

This allows a current bundle to complete even though it needs to go
through polling to do so, but it will not allow another bundle to be
started once that has happened. Ideally we would abort a bundle if it
was only partially sent, but as some parts of it already went out on the
wire, this obviously isn't feasible. Not continuing more bundle attempts
post encountering a full socket buffer is the second best thing.

e7c30675a7 io_uring/bpf_filter: cache lookup table in ctx->bpf_filters
Currently a few pointer dereferences need to be made to both check if
BPF filters are installed, and then also to retrieve the actual filter
for the opcode. Cache the table in ctx->bpf_filters to avoid that.

Add a bit of debug info on ring exit to show if we ever got this wrong.
Small risk of that given that the table is currently only updated in one
spot, but once task forking is enabled, that will add one more spot.

Add support for loading classic BPF programs with io_uring to provide
fine-grained filtering of SQE operations. Unlike
IORING_REGISTER_RESTRICTIONS which only allows bitmap-based allow/deny
of opcodes, BPF filters can inspect request attributes and make dynamic
decisions.

The filter is registered via IORING_REGISTER_BPF_FILTER with a struct

  - Return 0 to deny the request with -EACCES
  - Multiple filters can be stacked per opcode. All filters must
    return 1 for the opcode to be allowed.
  - Filters are evaluated in registration order (most recent first)

The implementation uses classic BPF (cBPF) rather than eBPF for as
that's required for containers, and since they can be used by any
user in the system.

795663b4d1 io_uring/zcrx: implement large rx buffer support
There are network cards that support receive buffers larger than 4K, and
that can be vastly beneficial for performance, and benchmarks for this
patch showed up to 30% CPU util improvement for 32K vs 4K buffers.

Allows zcrx users to specify the size in struct
io_uring_zcrx_ifq_reg::rx_buf_len. If set to zero, zcrx will use a
default value. zcrx will check and fail if the memory backing the area
can't be split into physically contiguous chunks of the required size.
It's more restrictive as it only needs dma addresses to be contig, but
that's beyond this series.

io_rsrc_node_lookup() takes a signed int index as input and compares it
to an unsigned length. Since the signed int is implicitly cast to an
unsigned int for the comparison and the length is bounded by
IORING_MAX_FIXED_FILES/IORING_MAX_REG_BUFFERS, negative indices are
already rejected on architectures where int is at least 32 bits. Make
this a bit clearer and avoid compiler warnings for comparisons of
signed and unsigned values by taking an unsigned int index instead.

5247c034a6 io_uring: introduce non-circular SQ
Outside of SQPOLL, normally SQ entries are consumed by the time the
submission syscall returns. For those cases we don't need a circular
buffer and the head/tail tracking, instead the kernel can assume that
entries always start from the beginning of the SQ at index 0. This patch
introduces a setup flag doing exactly that. It's a simpler and helps
to keeps SQEs hot in cache.

The feature is optional and enabled by setting IORING_SETUP_SQ_REWIND.
The flag is rejected if passed together with SQPOLL as it'd require
waiting for SQ before each submission. It also requires
IORING_SETUP_NO_SQARRAY, which can be supported but it's unlikely there
will be users, so leave more space for future optimisations.

Use the same trick that blk_io_schedule() does to avoid triggering the
hung task warning (and potential reboot/panic, depending on system
settings), and only wait for half the hung task timeout at the time.
If we exceed the default IO_URING_EXIT_WAIT_MAX period where we expect
things to certainly have finished unless there's a bug, then throw a
WARN_ON_ONCE() for that case.

f7bc22ca0d nvme/io_uring: optimize IOPOLL completions for local ring context
When multiple io_uring rings poll on the same NVMe queue, one ring can
find completions belonging to another ring. The current code always
uses task_work to handle this, but this adds overhead for the common
single-ring case.

This patch passes the polling io_ring_ctx through io_comp_batch's new
poll_ctx field. In io_do_iopoll(), the polling ring's context is stored
in iob.poll_ctx before calling the iopoll callbacks.

In nvme_uring_cmd_end_io(), we now compare iob->poll_ctx with the
request's owning io_ring_ctx (via io_uring_cmd_ctx_handle()). If they
match (local context), we complete inline with io_uring_cmd_done32().
If they differ (remote context) or iob is NULL (non-iopoll path), we
use task_work as before.

This optimization eliminates task_work scheduling overhead for the
common case where a ring polls and finds its own completions.

~10% IOPS improvement is observed in the following benchmark:

fio/t/io_uring -b512 -d128 -c32 -s32 -p1 -F1 -O0 -P1 -u1 -n1 /dev/ng0n1

will both end up calling io_flush_timeouts() and at least one of them
will see the correct value.

10dc959398 io_uring/io-wq: check IO_WQ_BIT_EXIT inside work run loop
Currently this is checked before running the pending work. Normally this
is quite fine, as work items either end up blocking (which will create a
new worker for other items), or they complete fairly quickly. But syzbot
reports an issue where io-wq takes seemingly forever to exit, and with a
bit of debugging, this turns out to be because it queues a bunch of big
(2GB - 4096b) reads with a /dev/msr* file. Since this file type doesn't
support ->read_iter(), loop_rw_iter() ends up handling them. Each read
returns 16MB of data read, which takes 20 (!!) seconds. With a bunch of
these pending, processing the whole chain can take a long time. Easily
longer than the syzbot uninterruptible sleep timeout of 140 seconds.
This then triggers a complaint off the io-wq exit path:

There's really nothing wrong here, outside of processing these reads
will take a LONG time. However, we can speed up the exit by checking the
IO_WQ_BIT_EXIT inside the io_worker_handle_work() loop, as syzbot will
exit the ring after queueing up all of these reads. Then once the first
item is processed, io-wq will simply cancel the rest. That should avoid
syzbot running into this complaint again.

If a read/write request goes through io_req_rw_cleanup() and has an
allocated iovec attached and fails to put to the rw_cache, then it may
end up with an unaccounted iovec pointer. Have io_rw_recycle() return

free a potential iovec or not.

697a5284ad io_uring: fix IOPOLL with passthrough I/O
A previous commit improving IOPOLL made an incorrect assumption that
task_work isn't used with IOPOLL. This can cause crashes when doing
passthrough I/O on nvme, where queueing the completion task_work will
trample on the same memory that holds the completed list of requests.

Fix it up by shuffling the members around, so we're not sharing any
parts that end up getting used in this path.

da579f05ef io_uring: move local task_work in exit cancel loop
With IORING_SETUP_DEFER_TASKRUN, task work is queued to ctx->work_llist
(local work) rather than the fallback list. During io_ring_exit_work(),
io_move_task_work_from_local() was called once before the cancel loop,
moving work from work_llist to fallback_llist.

However, task work can be added to work_llist during the cancel loop
itself. There are two cases:

1) io_kill_timeouts() is called from io_uring_try_cancel_requests() to
cancel pending timeouts, and it adds task work via io_req_queue_tw_complete()
for each cancelled timeout:

2) URING_CMD requests like ublk can be completed via
io_uring_cmd_complete_in_task() from ublk_queue_rq() during canceling,
given ublk request queue is only quiesced when canceling the 1st uring_cmd.

Since io_allowed_defer_tw_run() returns false in io_ring_exit_work()
(kworker != submitter_task), io_run_local_work() is never invoked,
and the work_llist entries are never processed. This causes
io_uring_try_cancel_requests() to loop indefinitely, resulting in
100% CPU usage in kworker threads.

Fix this by moving io_move_task_work_from_local() inside the cancel
loop, ensuring any work on work_llist is moved to fallback before
each cancel attempt.

9fa3ec8458 allow incomplete imports of filenames
There are two filename-related problems in io_uring and its
interplay with audit.

Filenames are imported when request is submitted and used when
it is processed.  Unfortunately, the latter may very well
happen in a different thread.  In that case the reference to
filename is put into the wrong audit_context - that of submitting
thread, not the processing one.  Audit logics is called by
the latter, and it really wants to be able to find the names
in audit_context current (== processing) thread.

Another related problem is the headache with refcounts -
normally all references to given struct filename are visible
only to one thread (the one that uses that struct filename).
io_uring violates that - an extra reference is stashed in
audit_context of submitter.  It gets dropped when submitter
returns to userland, which can happen simultaneously with
processing thread deciding to drop the reference it got.

We paper over that by making refcount atomic, but that means
pointless headache for everyone.

Solution: the notion of partially imported filenames.  Namely,
already copied from userland, but *not* exposed to audit yet.

io_uring can create that in submitter thread, and complete the
import (obtaining the usual reference to struct filename) in
processing thread.

Object: struct delayed_filename.

Primitives for working with it:

delayed_getname(&delayed_filename, user_string) - copies the name from
userland, returning 0 and stashing the address of (still incomplete)
struct filename in delayed_filename on success and returning -E... on
error.

delayed_getname_uflags(&delayed_filename, user_string, atflags) -
similar, in the same relation to delayed_getname() as getname_uflags()
is to getname()

complete_getname(&delayed_filename) - completes the import of filename
stashed in delayed_filename and returns struct filename to caller,
emptying delayed_filename.

CLASS(filename_complete_delayed, name)(&delayed_filename) - variant of
CLASS(filename) with complete_getname() for constructor.

dismiss_delayed_filename(&delayed_filename) - destructor; drops whatever
might be stashed in delayed_filename, emptying it.

putname_to_delayed(&delayed_filename, name) - if name is shared, stashes
its copy into delayed_filename and drops the reference to name, otherwise
stashes the name itself in there.

__io_msg_ring_data() checks that the target_ctx isn't
IORING_SETUP_R_DISABLED before calling io_msg_data_remote(), which calls
io_msg_remote_post(). So submitter_task can't be modified concurrently
with the read in io_msg_remote_post(). Additionally, submitter_task must
exist, as io_msg_data_remote() is only called for io_msg_need_remote(),
i.e. task_complete is set, which requires IORING_SETUP_DEFER_TASKRUN,
which in turn requires IORING_SETUP_SINGLE_ISSUER. And submitter_task is
assigned in io_uring_create() or io_register_enable_rings() before
enabling any IORING_SETUP_SINGLE_ISSUER io_ring_ctx.
Similarly, io_msg_send_fd() checks IORING_SETUP_R_DISABLED and
io_msg_need_remote() before calling io_msg_fd_remote(). submitter_task
therefore can't be modified concurrently with the read in
io_msg_fd_remote() and must be non-null.
io_register_enable_rings() can't run concurrently because it's called
from io_uring_register() -> __io_uring_register() with uring_lock held.
Thus, replace the READ_ONCE() and WRITE_ONCE() of submitter_task with
plain loads and stores. And remove the NULL checks of submitter_task in
io_msg_remote_post() and io_msg_fd_remote().

15f506a77a io_uring: remove nr_segs recalculation in io_import_kbuf()
io_import_kbuf() recalculates iter->nr_segs to reflect only the bvecs
needed for the requested byte range. This was added to provide an
accurate segment count to bio_iov_bvec_set(), which copied nr_segs to
bio->bi_vcnt for use as a bio split hint.

The previous two patches eliminated this dependency:
 - bio_may_need_split() now uses bi_iter instead of bi_vcnt for split
   decisions
 - bio_iov_bvec_set() no longer copies nr_segs to bi_vcnt

Since nr_segs is no longer used for bio split decisions, the
recalculation loop is unnecessary. The iov_iter already has the correct
bi_size to cap iteration, so an oversized nr_segs is harmless.

e0392a10c9 io_uring/io-wq: fix incorrect io_wq_for_each_worker() termination logic
A previous commit added this helper, and had it terminate if false is
returned from the handler. However, that is completely opposite, it
should abort the loop if true is returned.

Fix this up by having io_wq_for_each_worker() keep iterating as long
as false is returned, and only abort if true is returned.

Add a separate lock just for the tctx_list, tctx_lock. This can nest
under ->uring_lock, where necessary, and be used separately for list

Allocate the overflowing CQE with GFP_NOWAIT instead of GFP_ATOMIC. This
changes causes allocations to fail earlier in out-of-memory situations,
rather than being deferred. Using GFP_ATOMIC allows a process to exceed
memory limits.

3c7d76d612 io_uring: IOPOLL polling improvements
io_uring manages issued and pending IOPOLL read/write requests in a
singly linked list. One downside of that is that individual items
cannot easily be removed from that list, and as a result, io_uring
will only complete a completed request N in that list if 0..N-1 are
also complete. For homogenous IO this isn't necessarily an issue,
but if different devices are involved in polling in the same ring, or
if disparate IO from the same device is being polled for, this can
defer completion of some requests unnecessarily.

Move to a doubly linked list for iopoll completions instead, making it
possible to easily complete whatever requests that were polled done
successfully.

b14fad5553 io_uring: fix filename leak in __io_openat_prep()
__io_openat_prep() allocates a struct filename using getname(). However,
for the condition of the file being installed in the fixed file table as
well as having O_CLOEXEC flag set, the function returns early. At that
point, the request doesn't have REQ_F_NEED_CLEANUP flag set. Due to this,
the memory for the newly allocated struct filename is not cleaned up,
causing a memory leak.

Fix this by setting the REQ_F_NEED_CLEANUP for the request just after the
successful getname() call, so that when the request is torn down, the
filename will be cleaned up, along with other resources needing cleanup.

io_import_kbuf() calculates nr_segs incorrectly when iov_offset is
non-zero after iov_iter_advance(). It doesn't account for the partial
consumption of the first bvec.

The problem comes when meet the following conditions:
1. Use UBLK_F_AUTO_BUF_REG feature of ublk.
2. The kernel will help to register the buffer, into the io uring.
3. Later, the ublk server try to send IO request using the registered
   buffer in the io uring, to read/write to fuse-based filesystem, with
O_DIRECT.

>From a userspace perspective, the ublk server thread is blocked in the
kernel, and will see "soft lockup" in the kernel dmesg.

When ublk registers a buffer with mixed-size bvecs like [4K]*6 + [12K]
and a request partially consumes a bvec, the next request's nr_segs
calculation uses bvec->bv_len instead of (bv_len - iov_offset).

This causes fuse_get_user_pages() to loop forever because nr_segs
indicates fewer pages than actually needed.

Specifically, the infinite loop happens at:
fuse_get_user_pages()
  -> iov_iter_extract_pages()
    -> iov_iter_extract_bvec_pages()
Since the nr_segs is miscalculated, the iov_iter_extract_bvec_pages
returns when finding that i->nr_segs is zero. Then
iov_iter_extract_pages returns zero. However, fuse_get_user_pages does
still not get enough data/pages, causing infinite loop.

Example:
  - Bvecs: [4K, 4K, 4K, 4K, 4K, 4K, 12K, ...]
  - Request 1: 32K at offset 0, uses 6*4K + 8K of the 12K bvec
  - Request 2: 32K at offset 32K
    - iov_offset = 8K (8K already consumed from 12K bvec)
    - Bug: calculates using 12K, not (12K - 8K) = 4K
    - Result: nr_segs too small, infinite loop in fuse_get_user_pages.

Fix by accounting for iov_offset when calculating the first segment's
available length.

Using min_wait, two timeouts are given:

1) The min_wait timeout, within which up to 'wait_nr' events are
   waited for.
2) The overall long timeout, which is entered if no events are generated
   in the min_wait window.

If the min_wait has expired, any event being posted must wake the task.
For SQPOLL, that isn't the case, as it won't trigger the io_has_work()
condition, as it will have already processed the task_work that happened
when an event was posted. This causes any event to trigger post the
min_wait to not always cause the waiting application to wakeup, and
instead it will wait until the overall timeout has expired. This can be
shown in a test case that has a 1 second min_wait, with a 5 second
overall wait, even if an event triggers after 1.5 seconds:

55d57b3bcc io_uring/poll: unify poll waitqueue entry and list removal
For some cases, the order in which the waitq entry list and head
writing happens is important, for others it doesn't really matter.
But it's somewhat confusing to have them spread out over the file.

Abstract out the nicely documented code in io_pollfree_wake() and
move it into a helper, and use that helper consistently rather than
having other call sites manually do the same thing. While at it,
correct a comment function name as well.

78385c7299 io_uring/kbuf: use READ_ONCE() for userspace-mapped memory
The struct io_uring_buf elements in a buffer ring are in a memory region
accessible from userspace. A malicious/buggy userspace program could
therefore write to them at any time, so they should be accessed with
READ_ONCE() in the kernel. Commit 98b6fa62c8 ("io_uring/kbuf: always
use READ_ONCE() to read ring provided buffer lengths") already switched
the reads of the len field to READ_ONCE(). Do the same for bid and addr.

48f22f8093 block: enable per-cpu bio cache by default
Since after commit 12e4e8c7ab ("io_uring/rw: enable bio caches for
IRQ rw"), bio_put is safe for task and irq context, bio_alloc_bioset is
safe for task context and no one calls in irq context, so we can enable
per cpu bio cache by default.

Benchmarked with t/io_uring and ext4+nvme:
taskset -c 6 /root/fio/t/io_uring  -p0 -d128 -b4096 -s1 -c1 -F1 -B1 -R1
-X1 -n1 -P1  /mnt/testfile
base IOPS is 562K, patch IOPS is 574K. The CPU usage of bio_alloc_bioset
decrease from 1.42% to 1.22%.

The worst case is allocate bio in CPU A but free in CPU B, still use
t/io_uring and ext4+nvme:
base IOPS is 648K, patch IOPS is 647K.

Also use fio test ext4/xfs with libaio/sync/io_uring on null_blk and
nvme, no obvious performance regression.

34c78b8610 io_uring/io-wq: always retry worker create on ERESTART*
If a task has a pending signal when create_io_thread() is called,
copy_process() will return -ERESTARTNOINTR. io_should_retry_thread()
will request a retry of create_io_thread() up to WORKER_INIT_LIMIT = 3
times. If all retries fail, the io_uring request will fail with
ECANCELED.
Commit 3918315c5dc ("io-wq: backoff when retrying worker creation")
added a linear backoff to allow the thread to handle its signal before
the retry. However, a thread receiving frequent signals may get unlucky
and have a signal pending at every retry. Since the userspace task
doesn't control when it receives signals, there's no easy way for it to
prevent the create_io_thread() failure due to pending signals. The task
may also lack the information necessary to regenerate the canceled SQE.
So always retry the create_io_thread() on the ERESTART* errors,
analogous to what a fork() syscall would do. EAGAIN can occur due to
various persistent conditions such as exceeding RLIMIT_NPROC, so respect
the WORKER_INIT_LIMIT retry limit for EAGAIN errors.

84230ad2d2 io_uring/poll: correctly handle io_poll_add() return value on update
When the core of io_uring was updated to handle completions
consistently and with fixed return codes, the POLL_REMOVE opcode
with updates got slightly broken. If a POLL_ADD is pending and
then POLL_REMOVE is used to update the events of that request, if that
update causes the POLL_ADD to now trigger, then that completion is lost
and a CQE is never posted.

Additionally, ensure that if an update does cause an existing POLL_ADD
to complete, that the completion value isn't always overwritten with
-ECANCELED. For that case, whatever io_poll_add() set the value to
should just be retained.

5d24321e4c io_uring: Introduce getsockname io_uring cmd
Introduce a socket-specific io_uring_cmd to support
getsockname/getpeername via io_uring.  I made this an io_uring_cmd
instead of a new operation to avoid polluting the command namespace with
what is exclusively a socket operation.  In addition, since we don't
need to conform to existing interfaces, this merges the
getsockname/getpeername in a single operation, since the implementation
is pretty much the same.

This has been frequently requested, for instance at [1] and more
recently in the project Discord channel. The main use-case is to support
fixed socket file descriptors.

f6dc5a3619 io_uring: fix mixed cqe overflow handling
I started to see zcrx data corruptions. That turned out to be due
to CQ tail pointing to a stale entry which happened to be from
a zcrx request. I.e. the tail is incremented without the CQE
memory being changed.

The culprit is __io_cqring_overflow_flush() passing "cqe32=true"
to io_get_cqe_overflow() for non-mixed CQE32 setups, which only
expects it to be set for mixed 32B CQEs and not for SETUP_CQE32.

The fix is slightly hacky, long term it's better to unify mixed and
CQE32 handling.

f6041803a8 io_uring/net: ensure vectored buffer node import is tied to notification
When support for vectored registered buffers was added, the import
itself is using 'req' rather than the notification io_kiocb, sr->notif.
For non-vectored imports, sr->notif is correctly used. This is important
as the lifetime of the two may be different. Use the correct io_kiocb
for the vectored buffer import.

46447367a5 io_uring/cmd_net: fix wrong argument types for skb_queue_splice()
If timestamp retriving needs to be retried and the local list of
SKB's already has entries, then it's spliced back into the socket
queue. However, the arguments for the splice helper are transposed,
causing exactly the wrong direction of splicing into the on-stack
list. Fix that up.

f779ac0b87 io_uring/register: use correct location for io_rings_layout
A previous consolidated the ring size etc calculations into
io_prepare_config(), but missed updating io_register_resize_rings()
correctly to use the calculated values. As a result, it ended up using
on-stack uninitialized values, and hence either failed validating the
size correctly, or just failed resizing because the sizes were random.

This caused failures in the liburing regression tests:

9ac09bb9fe mm: consistently use current->mm in mm_get_unmapped_area()
mm_get_unmapped_area() is a wrapper around arch_get_unmapped_area() /
arch_get_unmapped_area_topdown(), both of which search current->mm for
some free space.  Neither take an mm_struct - they implicitly operate on
current->mm.

But the wrapper takes an mm_struct and uses it to decide whether to search
bottom up or top down.  All callers pass in current->mm for this, so
everything is working consistently.  But it feels like an accident waiting
to happen; eventually someone will call that function with a different mm,
expecting to find free space in it, but what gets returned is free space
in the current mm.

So let's simplify by removing the parameter and have the wrapper use
current->mm to decide which end to start at.  Now everything is consistent
and self-documenting.

Add a way to share an ifq from a src ring that is real (i.e. bound to a
HW RX queue) with other rings. This is done by passing a new flag
IORING_ZCRX_IFQ_REG_IMPORT in the registration struct
io_uring_zcrx_ifq_reg, alongside the fd of an exported zcrx ifq.

d7af80b213 io_uring/zcrx: export zcrx via a file
Add an option to wrap a zcrx instance into a file and expose it to the
user space. Currently, users can't do anything meaningful with the file,
but it'll be used in a next patch to import it into another io_uring
instance. It's implemented as a new op called ZCRX_CTRL_EXPORT for the
IORING_REGISTER_ZCRX_CTRL registration opcode.

39c9676f78 io_uring/zcrx: count zcrx users
zcrx tries to detach ifq / terminate page pools when the io_uring ctx
owning it is being destroyed. There will be multiple io_uring instances
attached to it in the future, so add a separate counter to track the
users. Note, refs can't be reused for this purpose as it only used to
prevent zcrx and rings destruction, and also used by page pools to keep
it alive.

475eb39b00 io_uring/zcrx: add sync refill queue flushing
Add an zcrx interface via IORING_REGISTER_ZCRX_CTRL that forces the
kernel to flush / consume entries from the refill queue. Just as with
the IORING_REGISTER_ZCRX_REFILL attempt, the motivation is to address
cases where the refill queue becomes full, and the user can't return
buffers and needs to stash them. It's still a slow path, and the user
should size refill queue appropriately, but it should be helpful for
handling temporary traffic spikes and other unpredictable conditions.

The interface is simpler comparing to ZCRX_REFILL as it doesn't need
temporary refill entry arrays and gives natural batching, whereas
ZCRX_REFILL requires even more user logic to be somewhat efficient.

Also, add a structure for the operation. It's not currently used but
can serve for future improvements like limiting the number of buffers to
process, etc.

zcrx sqe->msg_flags has never been defined and checked to be zero. It
doesn't need to be a MSG_* bitmask. Keep them undefined, don't mix

Same problem as with zcrx in the previous patch, the user needs to know
SQ/CQ header sizes to allocated memory before setup to use it for user
provided rings, i.e. IORING_SETUP_NO_MMAP, however that information is
only returned after registration, hence the user is guessing kernel
implementation details.

Return the header size and alignment, which is split with the same
motivation, to allow the user to know the real structure size without
alignment in case there will be more flexible placement schemes in the
future.

2647e2ecc0 io_uring/query: introduce zcrx query
Add a new query type IO_URING_QUERY_ZCRX returning the user some basic
information about the interface, which includes allowed flags for areas
and registration and supported IORING_REGISTER_ZCRX_CTRL subcodes.

There is also a chicken-egg problem with user provided refill queue
memory, where offsets and size information is returned after
registration, but to properly allocate memory you need to know it
beforehand, which is why the userspace currently has to guess the RQ
headers size and severely overestimates it. Return the size information.
It's split into "size" and "alignment" fields because for default
placement modes the user is interested in the aligned size, however if
it gets support for more flexible placement, it'll need to only know the
actual header size.

eb76ff6a68 io_uring: pre-calculate scq layout
Move ring layouts calculations into io_prepare_config(), so that more
misconfiguration checking can be done earlier before creating a ctx.
It also deduplicates some code with ring resizing. And as a bonus, now
it initialises params->sq_off.array, which is closer to all other user
offset init, and also applies it to ring resizing, which was previously
missing it.

There will be more information needed during ctx setup, and instead of
passing a handful of pointers around, wrap them all into a new
structure. Add a helper for encapsulating all configuration checks and
preparation, that's also reused for ring resizing.

Note, it indirectly adds a io_uring_sanitise_params() check to ring
resizing, which is a good thing.
