dbbca20764 blk-cgroup: fix leaks and online flag on radix_tree_insert failure
When radix_tree_insert() fails in blkg_create(), the error path has two
issues:

1. blkg->online is set to true unconditionally, even when the blkg was
   never fully inserted.  Move the assignment inside the success block.

2. The error path calls blkg_put() without first calling
   percpu_ref_kill().  Because the refcount is still in percpu mode,
   percpu_ref_put() only does this_cpu_sub() without checking for zero,
   so blkg_release() is never triggered.  This permanently leaks the
   blkg memory, its percpu iostat, policy data, the parent blkg
   reference, and the cgroup css reference — the latter preventing the
   cgroup from ever being destroyed.

Fix by replacing blkg_put() with percpu_ref_kill(), matching the pattern
used in blkg_destroy().

4cfd7c1cff blk-cgroup: don't nest queue_lock under blkcg->lock in blkcg_destroy_blkgs()
The correct lock order is q->queue_lock before blkcg->lock, and in order
to prevent deadlock from blkcg_destroy_blkgs(), trylock is used for
q->queue_lock while blkcg->lock is already held, this is hacky.

Refactor blkcg_destroy_blkgs() to hold blkcg->lock only long enough to
get the first blkg and then release it. Then take q->queue_lock and
blkcg->lock in the correct order to destroy the blkg. This is a very cold
path, so the extra lock/unlock cycles are acceptable.

Also prepare to convert protecting blkcg with blkcg_mutex instead of
queue_lock.

457d3c4f0f blk-cgroup: don't nest queue_lock under rcu in bio_associate_blkg()
If a bio is already associated with a blkg, the blkcg is already pinned
until the bio is done, so there is no need for RCU protection. Otherwise,
protect blkcg_css() with RCU independently. Prepare to protect blkcg with
blkcg_mutex instead of queue_lock.

56cc24f59c blk-cgroup: don't nest queue_lock under rcu in blkcg_print_blkgs()
With previous modification to delay freeing policy data after an RCU grace
period, prfill() can run under RCU instead of taking queue_lock. However,
policy teardown can still clear blkg->pd[plid] after blkcg_print_blkgs()
observes the policy enabled bit.

Load policy data once with READ_ONCE() and skip the blkg if teardown
already cleared it. Do the same in recursive stat walks for descendant
blkgs. Remove the stale BFQ debug queue_lock assertion because
blkcg_print_blkgs() no longer calls prfill() with queue_lock held. This
also lets ioc_qos_prfill() and ioc_cost_model_prfill() use IRQ-safe
ioc->lock locking without re-enabling IRQs while queue_lock is still held.

25656304da blk-cgroup: protect iterating blkgs with blkcg->lock in blkcg_print_stat()
blkcg_print_one_stat() will be called for each blkg:
- access blkg->iostat, which is freed from rcu callback
  blkg_free_workfn();
- access policy data from pd_stat_fn(), which is freed from
  pd_free_fn(), while pd_free_fn() can be called by removing blkcg or
  deactivating policy;

Take blkcg->lock while iterating so the blkgs stay online and both
blkg->iostat and policy data for activated policies stay valid.  Use
irq-safe locking because blkcg->lock can be nested under q->queue_lock,
which is used from IRQ completion paths.

Prepare to convert protecting blkgs from request_queue with mutex.

As a result, a blkg can still be reachable through q->blkg_list while
its ->blkcg has already been freed.

[Fix]
Fix this by deferring the blkcg css_put() until after the blkg has been
unlinked from q->blkg_list in blkg_free_workfn(). This ensures that the
blkcg outlives every blkg still reachable through q->blkg_list, so any
iterator holding q->queue_lock is guaranteed to observe a valid
blkg->blkcg.

While at it, move css_tryget_online() from blkg_create() into blkg_alloc()
so that the css reference is owned by the alloc/free pair rather than
straddling layers:

0ab5ee5a1b blk-cgroup: fix UAF in __blkcg_rstat_flush()
When multiple blkgs in the same blkcg are released concurrently,
a use-after-free can occur. The race happens when one blkg's
__blkcg_rstat_flush() removes another blkg's iostat entries via
llist_del_all(). The second blkg sees an empty list and proceeds
to free itself while the first is still iterating over its entries.

Move the flush from __blkg_release() (RCU callback) to blkg_release()
(before call_rcu). This ensures the RCU grace period waits for any
concurrent flush's rcu_read_lock() section to complete before freeing.

998cda78d4 block/cgroup: Inline blkg_conf_{open,close}_bdev_frozen()
The blkg_conf_open_bdev_frozen() calling convention is not compatible
with lock context annotations. Fold both blkg_conf_open_bdev_frozen()
and blkg_conf_close_bdev_frozen() into their only caller. This patch
prepares for enabling lock context analysis.

The type of 'memflags' has been changed from unsigned long into unsigned
int to match the type of current->flags. See also <linux/sched.h>.

Split blkg_conf_exit() into blkg_conf_unprep() and blkg_conf_close_bdev()
because blkg_conf_exit() is not compatible with the Clang thread-safety
annotations. Remove blkg_conf_exit(). Rename blkg_conf_exit_frozen() into
blkg_conf_close_bdev_frozen(). Add thread-safety annotations to the new
functions.

Move the blkg_conf_open_bdev() call out of blkg_conf_prep() to make it
possible to add lock context annotations to blkg_conf_prep(). Change an
if-statement in blkg_conf_open_bdev() into a WARN_ON_ONCE() call. Export
blkg_conf_open_bdev() because it is called by the BFQ I/O scheduler and
the BFQ I/O scheduler may be built as a kernel module.

8817005efb cgroup/rstat: validate cpu before css_rstat_cpu() access
css_rstat_updated() is exposed as a BPF kfunc and accepts a
caller-provided cpu argument. The function uses cpu for per-cpu rstat
lookups without checking whether it refers to a valid possible CPU.

A BPF iter/cgroup program with CAP_BPF and CAP_PERFMON can pass an
invalid cpu value. On an unfixed UBSCAN_BOUNDS test kernel, cpu ==
0x7fffffff triggers:

  UBSAN: array-index-out-of-bounds in kernel/cgroup/rstat.c:31:9
  index 2147483647 is out of range for type 'long unsigned int [64]'

23308af722 blk-cgroup: fix disk reference leak in blkcg_maybe_throttle_current()
Add the missing put_disk() on the error path in
blkcg_maybe_throttle_current(). When blkcg lookup, blkg lookup, or
blkg_tryget() fails, the function jumps to the out label which only
calls rcu_read_unlock() but does not release the disk reference acquired
by blkcg_schedule_throttle() via get_device(). Since current->throttle_disk
is already set to NULL before the lookup, blkcg_exit() cannot release
this reference either, causing the disk to never be freed.

Restore the reference release that was present as blk_put_queue() in the
original code but was inadvertently dropped during the conversion from
request_queue to gendisk.

3dbaacf6ab blk-cgroup: wait for blkcg cleanup before initializing new disk
When a queue is shared across disk rebind (e.g., SCSI unbind/bind), the
previous disk's blkcg state is cleaned up asynchronously via
disk_release() -> blkcg_exit_disk(). If the new disk's blkcg_init_disk()
runs before that cleanup finishes, we may overwrite q->root_blkg while
the old one is still alive, and radix_tree_insert() in blkg_create()
fails with -EEXIST because the old blkg entries still occupy the same
queue id slot in blkcg->blkg_tree. This causes the sd probe to fail
with -ENOMEM.

Fix it by waiting in blkcg_init_disk() for root_blkg to become NULL,
which indicates the previous disk's blkcg cleanup has completed.

bf4afc53b7 Convert 'alloc_obj' family to use the new default GFP_KERNEL argument
This was done entirely with mindless brute force, using

    git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
        xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'

to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.

Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.

For the same reason the 'flex' versions will be done as a separate
conversion.

69050f8d6d treewide: Replace kmalloc with kmalloc_obj for non-scalar types
This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with:	kmalloc_flex(*PTR, FAM, COUNT, ...)

(where TYPE may also be *VAR)

The resulting allocations no longer return "void *", instead returning
"TYPE *".

08823e89e3 block: Remove elevator_lock usage from blkg_conf frozen operations
Remove the acquisition and release of q->elevator_lock in the
blkg_conf_open_bdev_frozen() and blkg_conf_exit_frozen() functions. The
elevator lock is no longer needed in these code paths since commit
78c271344b ("block: move wbt_enable_default() out of queue freezing
from sched ->exit()") which introduces `disk->rqos_state_mutex` for
protecting wbt state change, and not necessary to abuse elevator_lock
for this purpose.

This change helps to solve the lockdep warning reported from Yu Kuai[1].

Pass blktests/throtl with lockdep enabled.

convert blk_queue_enter() to blkcg_mutex to fix this problem. Meanwhile,
consider that blkcg_mutex is held after queue is freezed from policy
deactivation, also convert blkg_alloc() to use GFP_NOIO.

bd9fd5be6b blk-throttle: fix access race during throttle policy activation
On repeated cold boots we occasionally hit a NULL pointer crash in
blk_should_throtl() when throttling is consulted before the throttle
policy is fully enabled for the queue. Checking only q->td != NULL is
insufficient during early initialization, so blkg_to_pd() for the
throttle policy can still return NULL and blkg_to_tg() becomes NULL,
which later gets dereferenced.

Tighten blk_throtl_activated() to also require that the throttle policy

748922dcfa cgroup: use subsystem-specific rstat locks to avoid contention
It is possible to eliminate contention between subsystems when
updating/flushing stats by using subsystem-specific locks. Let the existing
rstat locks be dedicated to the cgroup base stats and rename them to
reflect that. Add similar locks to the cgroup_subsys struct for use with
individual subsystems.

Lock initialization is done in the new function ss_rstat_init(ss) which
replaces cgroup_rstat_boot(void). If NULL is passed to this function, the
global base stat locks will be initialized. Otherwise, the subsystem locks
will be initialized.

Change the existing lock helper functions to accept a reference to a css.
Then within these functions, conditionally select the appropriate locks
based on the subsystem affiliation of the given css. Add helper functions
for this selection routine to avoid repeated code.

a97915559f cgroup: change rstat function signatures from cgroup-based to css-based
This non-functional change serves as preparation for moving to
subsystem-based rstat trees. To simplify future commits, change the
signatures of existing cgroup-based rstat functions to become css-based and
rename them to reflect that.

Though the signatures have changed, the implementations have not. Within
these functions use the css->cgroup pointer to obtain the associated cgroup
and allow code to function the same just as it did before this patch. At
applicable call sites, pass the subsystem-specific css pointer as an
argument or pass a pointer to cgroup::self if not in subsystem context.

Note that cgroup_rstat_updated_list() and cgroup_rstat_push_children()
are not altered yet since there would be a larger amount of css to
cgroup conversions which may overcomplicate the code at this
intermediate phase.

The commit '245618f8e45f ("block: protect wbt_lat_usec using q->
elevator_lock")' introduced q->elevator_lock to protect updates
to blk-wbt parameters when writing to the sysfs attribute wbt_
lat_usec and the cgroup attribute io.cost.qos.  However, both
these attributes also acquire q->rq_qos_mutex, leading to the

>From the above log it's apparent that method which writes to sysfs attr
wbt_lat_usec acquires q->elevator_lock first, and then acquires q->rq_
qos_mutex. However the another method which writes to io.cost.qos,
acquires q->rq_qos_mutex first, and then acquires q->rq_qos_mutex. So
this could potentially cause the deadlock.

A closer look at ioc_qos_write shows that correcting the lock order is
non-trivial because q->rq_qos_mutex is acquired in blkg_conf_open_bdev
and released in blkg_conf_exit. The function blkg_conf_open_bdev is
responsible for parsing user input and finding the corresponding block
device (bdev) from the user provided major:minor number.

Since we do not know the bdev until blkg_conf_open_bdev completes, we
cannot simply move q->elevator_lock acquisition before blkg_conf_open_
bdev. So to address this, we intoduce new helpers blkg_conf_open_bdev_
frozen and blkg_conf_exit_frozen which are just wrappers around blkg_
conf_open_bdev and blkg_conf_exit respectively. The helper blkg_conf_
open_bdev_frozen is similar to blkg_conf_open_bdev, but additionally
freezes the queue, acquires q->elevator_lock and ensures the correct
locking order is followed between q->elevator_lock and q->rq_qos_mutex.
Similarly another helper blkg_conf_exit_frozen in addition to unfreezing
the queue ensures that we release the locks in correct order.

By using these helpers, now we maintain the same locking order in all
code paths where we update blk-wbt parameters.

e1a0202c6b blk-cgroup: improve policy registration error handling
This patch improve the returned error code of blkcg_policy_register().

1. Move the validation check for cpd/pd_alloc_fn and cpd/pd_free_fn
   function pairs to the start of blkcg_policy_register(). This ensures
   we immediately return -EINVAL if the function pairs are not correctly
   provided, rather than returning -ENOSPC after locking and unlocking
   mutexes unnecessarily.

   Those locks should not contention any problems, as error of policy
   registration is a super cold path.

2. Return -ENOMEM when cpd_alloc_fn() failed.

1e1a9cecfa block: force noio scope in blk_mq_freeze_queue
When block drivers or the core block code perform allocations with a
frozen queue, this could try to recurse into the block device to
reclaim memory and deadlock.  Thus all allocations done by a process
that froze a queue need to be done without __GFP_IO and __GFP_FS.
Instead of tying to track all of them down, force a noio scope as
part of freezing the queue.

Note that nvme is a bit of a mess here due to the non-owner freezes,
and they will be addressed separately.

d1248436cb blk-cgroup: Fix class @block_class's subsystem refcount leakage
blkcg_fill_root_iostats() iterates over @block_class's devices by
class_dev_iter_(init|next)(), but does not end iterating with
class_dev_iter_exit(), so causes the class's subsystem refcount leakage.

Fix by ending the iterating with class_dev_iter_exit().

Note that the UAF is not easy to trigger as the free path is indirected
behind a couple RCU grace periods and a work item execution. I could only

79c6c60a6c blk-ioprio: remove per-disk structure
ioprio works on the blk-cgroup level, all disks in the same cgroup
are the same, and the struct ioprio_blkg doesn't have anything in it.
Hence register the policy is enough, because cpd_alloc/free_fn will
be handled for each blk-cgroup, and there is no need to activate the
policy for disk. Hence remove blk_ioprio_init/exit and
ioprio_alloc/free_pd.

9d230c0996 blk-cgroup: Properly propagate the iostat update up the hierarchy
During a cgroup_rstat_flush() call, the lowest level of nodes are flushed
first before their parents. Since commit 3b8cc62987 ("blk-cgroup:
Optimize blkcg_rstat_flush()"), iostat propagation was still done to
the parent. Grandparent, however, may not get the iostat update if the
parent has no blkg_iostat_set queued in its lhead lockless list.

Fix this iostat propagation problem by queuing the parent's global
blkg->iostat into one of its percpu lockless lists to make sure that
the delta will always be propagated up to the grandparent and so on
toward the root blkcg.

Note that successive calls to __blkcg_rstat_flush() are serialized by
the cgroup_rstat_lock. So no special barrier is used in the reading
and writing of blkg->iostat.lqueued.

d0aac23635 blk-cgroup: fix list corruption from reorder of WRITE ->lqueued
__blkcg_rstat_flush() can be run anytime, especially when blk_cgroup_bio_start
is being executed.

If WRITE of `->lqueued` is re-ordered with READ of 'bisc->lnode.next' in
the loop of __blkcg_rstat_flush(), `next_bisc` can be assigned with one
stat instance being added in blk_cgroup_bio_start(), then the local
list in __blkcg_rstat_flush() could be corrupted.

Fix the issue by adding one barrier.

Merge tag 'for-6.10/block-20240511' of git://git.kernel.dk/linux

Pull block updates from Jens Axboe:

 - Add a partscan attribute in sysfs, fixing an issue with systemd
   relying on an internal interface that went away.

 - Attempt #2 at making long running discards interruptible. The
   previous attempt went into 6.9, but we ended up mostly reverting it
   as it had issues.

 - Remove old ida_simple API in bcache

 - Support for zoned write plugging, greatly improving the performance
   on zoned devices.

 - Remove the old throttle low interface, which has been experimental
   since 2017 and never made it beyond that and isn't being used.

 - Remove page->index debugging checks in brd, as it hasn't caught
   anything and prepares us for removing in struct page.

 - MD pull request from Song

 - Don't schedule block workers on isolated CPUs

a3166c5170 blk-throttle: delay initialization until configuration
Other cgroup policy like bfq, iocost are lazy-initialized when they are
configured for the first time for the device, but blk-throttle is
initialized unconditionally from blkcg_init_disk().

Delay initialization of blk-throttle as well, to save some cpu and
memory overhead if it's not configured.

Noted that once it's initialized, it can't be destroyed until disk
removal, even if it's disabled.

8b8ace0803 block: fix q->blkg_list corruption during disk rebind
Multiple gendisk instances can allocated/added for single request queue
in case of disk rebind. blkg may still stay in q->blkg_list when calling
blkcg_init_disk() for rebind, then q->blkg_list becomes corrupted.

Fix the list corruption issue by:

- add blkg_init_queue() to initialize q->blkg_list & q->blkcg_mutex only
- move calling blkg_init_queue() into blk_alloc_queue()

The list corruption should be started since commit f1c006f1c6 ("blk-cgroup:
synchronize pd_free_fn() from blkg_free_workfn() and blkcg_deactivate_policy()")
which delays removing blkg from q->blkg_list into blkg_free_workfn().

08420cf70c block: add blk_time_get_ns() and blk_time_get() helpers
Convert any user of ktime_get_ns() to use blk_time_get_ns(), and
ktime_get() to blk_time_get(), so we have a unified API for querying the
current time in nanoseconds or as ktime.

No functional changes intended, this patch just wraps ktime_get_ns()
and ktime_get() with a block helper.

521277d12b block: Correct a documentation comment in blk-cgroup.c
Commit 99e6038743
("blk-cgroup: pass a gendisk to the blkg allocation helpers") changed
blkg_alloc() to take a struct gendisk instead of a struct request_queue,
but the documentation comment still referred to q.

So, update that comment to refer to disk instead and fix a typo.

fab4c16c52 blk-cgroup: don't use removal safe list iterators
Commit f1c006f1c6 moved deletion of the list blkg->q_node from
blkg_destroy() to blkg_free_workfn(). Switch to using the list
iterators, as we don't need removal protection anymore.
