37953cec77 nvme: fix race condition between connected uevent and STARTED_ONCE flag
When a controller connects, nvme_start_ctrl() emits the
"NVME_EVENT=connected" uevent and sets the NVME_CTRL_STARTED_ONCE flag.
Currently, the uevent is emitted before the flag is set. This creates a
race condition for userspace tools (like udev rules) that might rely on
the "connected" event to configure other attributes.

Swap the order of operations in nvme_start_ctrl() so that the
NVME_CTRL_STARTED_ONCE flag is set before the uevent is sent. This
guarantees that the admin_timeout can already be changed when userspace
is notified.

The warning is a bit alarming, and it only prints for the very first
non-sgl capable device that receives a passthrough command. Just log an
informational message on initial discovery for every device.

40f0496b61 nvme: respect NVME_QUIRK_DISABLE_WRITE_ZEROES when wzsl is set
The NVM Command Set Identify Controller data may report a non-zero
Write Zeroes Size Limit (wzsl). When present, nvme_init_non_mdts_limits()
unconditionally overrides max_zeroes_sectors from wzsl, even if
NVME_QUIRK_DISABLE_WRITE_ZEROES previously set it to zero.

This effectively re-enables write zeroes for devices that need it
disabled, defeating the quirk. Several Kingston OM* drives rely on
this quirk to avoid firmware issues with write zeroes commands.

Check for the quirk before applying the wzsl override.

1029298da3 nvme: set discard_granularity from NPDG/NPDA
Currently, nvme_config_discard() always sets the discard_granularity
queue limit to the logical block size. However, NVMe namespaces can
advertise a larger preferred discard granularity in the NPDG or NPDA
field of the Identify Namespace structure or the NPDGL or NPDAL fields
of the I/O Command Set Specific Identify Namespace structure.

Use these fields to compute the discard_granularity limit. The logic is
somewhat involved. First, the fields are optional. NPDG is only reported
if the low bit of OPTPERF is set in NSFEAT. NPDA is reported if any bit
of OPTPERF is set. And NPDGL and NPDAL are reported if the high bit of
OPTPERF is set. NPDGL and NPDAL can also each be set to 0 to opt out of
reporting a limit. I/O Command Set Specific Identify Namespace may also
not be supported by older NVMe controllers. Another complication is that
multiple values may be reported among NPDG, NPDGL, NPDA, and NPDAL. The
spec says to prefer the values reported in the L variants. The spec says
NPDG should be a multiple of NPDA and NPDGL should be a multiple of
NPDAL, but it doesn't specify a relationship between NPDG and NPDAL or
NPDGL and NPDA. So use the maximum of the reported NPDG(L) and NPDA(L)
values as the discard_granularity.

823340b7e8 nvme: always issue I/O Command Set specific Identify Namespace
Currently, the I/O Command Set specific Identify Namespace structure is
only fetched for controllers that support extended LBA formats. This is
because struct nvme_id_ns_nvm is only used by nvme_configure_pi_elbas(),
which is only called when the ELBAS bit is set in the CTRATT field of
the Identify Controller structure.

However, the I/O Command Set specific Identify Namespace structure will
soon be used in nvme_update_disk_info(), so always try to obtain it in
nvme_update_ns_info_block(). This Identify structure is first defined in
NVMe spec version 2.0, but controllers reporting older versions could
still implement it.

d3c04a6ea5 nvme: update nvme_id_ns OPTPERF constants
In NVMe verson 2.0 and below, OPTPERF comprises only bit 4 of NSFEAT in
the Identify Namespace structure. Since version 2.1, OPTPERF includes
both bits 4 and 5 of NSFEAT. Replace the NVME_NS_FEAT_IO_OPT constant
with NVME_NS_FEAT_OPTPERF_SHIFT, NVME_NS_FEAT_OPTPERF_MASK, and
NVME_NS_FEAT_OPTPERF_MASK_2_1, representing the first bit, pre-2.1 bit
width, and post-2.1 bit width of OPTPERF.

Update nvme_update_disk_info() to check both OPTPERF bits for
controllers that report version 2.1 or newer, as NPWG and NOWS are
supported even if only bit 5 is set.

The choice of what queue limits are set in nvme_update_disk_info() vs.
nvme_config_discard() seems a bit arbitrary. A subsequent commit will
compute the discard_granularity limit using struct nvme_id_ns, which is
only passed to nvme_update_disk_info() currently. So move the logic in
nvme_config_discard() to nvme_update_disk_info(). Replace several
instances of ns->ctrl in nvme_update_disk_info() with the ctrl variable
brought from nvme_config_discard().

203247c5cb blk-integrity: support arbitrary buffer alignment
A bio segment may have partial interval block data with the rest
continuing into the next segments because direct-io data payloads only
need to align in memory to the device's DMA limits.

At the same time, the protection information may also be split in
multiple segments. The most likely way that may happen is if two
requests merge, or if we're directly using the io_uring user metadata.
The generate/verify, however, only ever accessed the first bip_vec.

Further, it may be possible to unalign the protection fields from the
user space buffer, or if there are odd additional opaque bytes in front
or in back of the protection information metadata region.

Change up the iteration to allow spanning multiple segments. This patch
is mostly a re-write of the protection information handling to allow any
arbitrary alignments, so it's probably easier to review the end result
rather than the diff.

Many controllers are not able to handle interval data composed of
multiple segments when PI is used, so this patch introduces a new
integrity limit that a low level driver can set to notify that it is
capable, default to false. The nvme driver is the first one to enable it
in this patch. Everyone else will force DMA alignment to the logical
block size as before to ensure interval data is always aligned within a
single segment.

0375c81eb2 nvme-core: do not pass empty queue_limits to blk_mq_alloc_queue()
In nvme_alloc_admin_tag_set(), an empty queue_limits struct is
currently allocated on the stack and passed by reference to
blk_mq_alloc_queue().

This is redundant because blk_mq_alloc_queue() already handles
a NULL limits pointer by internally substituting it with a default
empty queue_limits struct.
Remove the unnecessary local variable and pass a NULL value.

This was done entirely with mindless brute force, using

to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.

Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.

For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with:	kmalloc_flex(*PTR, FAM, COUNT, ...)

(where TYPE may also be *VAR)

The resulting allocations no longer return "void *", instead returning
"TYPE *".

head"), we stopped using the namespace to hold the passthrough logging
enabled attribute. There is now nowhere now which looks up the gendisk dev
driver data, so stop setting it.

Incidentally, it would have been better to set this before adding the
disk.

ac30cd3043 nvme: stop using AWUPF
As described at [0], much of the atomic write parts of the specification
are lacking.

For now, there is nothing which we can do in software about the lack of
a dedicated NVMe write atomic command.

As for reading the atomic write limits, it is felt that the per-namespace
values are mostly properly specified and it is assumed that they are
properly implemented.

The specification of NAWUPF is quite clear. However the specification of
NABSPF is less clear. The lack of clarity in NABSPF comes from deciding
whether NABSPF applies when NSABP is 0 - it is assumed that NSABPF does
not apply when NSABP is 0.

As for the per-controller AWUPF, how this value applies to shared
namespaces is missing in the specification. Furthermore, the value is in
terms of logical blocks, which is an NS entity.

Since AWUPF is so poorly defined, stop using it already together.
Hopefully this will force vendors to implement NAWUPF support always.

Note that AWUPF not only effects atomic write support, but also the
physical block size reported for the device.

To help users know this restriction, log an info message per NS.

5e2fde1a94 block: pass io_comp_batch to rq_end_io_fn callback
Add a third parameter 'const struct io_comp_batch *' to the rq_end_io_fn
callback signature. This allows end_io handlers to access the completion
batch context when requests are completed via blk_mq_end_request_batch().

The io_comp_batch is passed from blk_mq_end_request_batch(), while NULL
is passed from __blk_mq_end_request() and blk_mq_put_rq_ref() which don't
have batch context.

This infrastructure change enables drivers to detect whether they're
being called from a batched completion path (like iopoll) and access
additional context stored in the io_comp_batch.

bc840b21a2 nvme: remove virtual boundary for sgl capable devices
The nvme virtual boundary is only required for the PRP format. Devices
that can use SGL for DMA don't need it for IO queues. Drop reporting it
for such devices; rdma fabrics controllers will continue to use the
limit as they currently don't report any boundary requirements, but tcp
and fc never needed it in the first place so they get to report no
virtual boundary.

Applications may continue to align to the same virtual boundaries for
optimization purposes if they want, and the driver will continue to
decide whether to use the PRP format the same as before if the IO allows
it.

However, this solution makes following the path of a zone information
harder to understand.

Clean this up by introducing the new blk_report_zones_args structure to
define a zone report callback and its private data and introduce the
helper function disk_report_zone() which calls both
disk_zone_wplug_sync_wp_offset() and the zone report user callback
function for all zones of a zone report. This helper function must be
called by all block device drivers that implement the report zones
block operation in order to correctly report a zone information.

All block device drivers supporting the report_zones block operation are
updated to use this new scheme.

7ac3c2889b nvme: fix PI insert on write
I recently ran into an issue where the PI generated using the block layer
integrity code differs from that from a kernel using the PRACT fallback
when the block layer integrity code is disabled, and I tracked this down
to us using PRACT incorrectly.

The NVM Command Set Specification (section 5.33 in 1.2, similar in older
versions) specifies the PRACT insert behavior as:

  Inserted protection information consists of the computed CRC for the
  protection information format (refer to section 5.3.1) in the Guard
  field, the LBAT field value in the Application Tag field, the LBST
  field value in the Storage Tag field, if defined, and the computed
  reference tag in the Logical Block Reference Tag.

Where the computed reference tag is defined as following for type 1 and
type 2 using the text below that is duplicated in the respective bullet
points:

  the value of the computed reference tag for the first logical block of
  the command is the value contained in the Initial Logical Block
  Reference Tag (ILBRT) or Expected Initial Logical Block Reference Tag
  (EILBRT) field in the command, and the computed reference tag is
  incremented for each subsequent logical block.

So we need to set ILBRT field, but we currently don't.  Interestingly
this works fine on my older type 1 formatted SSD, but Qemu trips up on
this.  We already set ILBRT for Write Same since commit aeb7bb061be5
("nvme: set the PRACT bit when using Write Zeroes with T10 PI").

To ease this, move the PI type check into nvme_set_ref_tag.

Instances are happier that way and it makes more sense anyway -
the only part of the result that is related to partition we are given
is the start sector, and that has been filled in by the caller.

Everything else is a function of the disk.  Only one instance
(DASD) is ever looking at anything other than bdev->bd_disk and
that one is trivial to adjust.

e715b8733d nvme: add capability to connect to an administrative controller
Add capability to connect to an administrative controller by
preventing ioq creation for admin-controllers.

Add a nvme_admin_ctrl() to check if a controller's CNTRLTYPE indicates
that it is an administrative controller and override ctrl->queue_count to
1 for admin controllers, so that only the admin queue and no I/O queues
are created for an administrative controller.  This override is done in
nvme_init_ctrl_finish() after ctrl->cntrltype has been initialized in
nvme_init_identify() so nvme_admin_ctrl() will work correctly.
Doing this override in generic code (nvme_init_ctrl_finish) makes it
transport agnostic and will work properly for nvme/tcp as well as for
nvme/rdma.

Pull fallocate updates from Christian Brauner:
 "fallocate() currently supports creating preallocated files
  efficiently. However, on most filesystems fallocate() will preallocate
  blocks in an unwriten state even if FALLOC_FL_ZERO_RANGE is specified.

  The extent state must later be converted to a written state when the
  user writes data into this range, which can trigger numerous metadata
  changes and journal I/O. This may leads to significant write
  amplification and performance degradation in synchronous write mode.

  At the moment, the only method to avoid this is to create an empty
  file and write zero data into it (for example, using 'dd' with a large
  block size). However, this method is slow and consumes a considerable
  amount of disk bandwidth.

  Now that more and more flash-based storage devices are available it is
  possible to efficiently write zeros to SSDs using the unmap write
  zeroes command if the devices do not write physical zeroes to the
  media.

  For example, if SCSI SSDs support the UMMAP bit or NVMe SSDs support
  the DEAC bit[1], the write zeroes command does not write actual data
  to the device, instead, NVMe converts the zeroed range to a
  deallocated state, which works fast and consumes almost no disk write
  bandwidth.

  This series implements the BLK_FEAT_WRITE_ZEROES_UNMAP feature and
  BLK_FLAG_WRITE_ZEROES_UNMAP_DISABLED flag for SCSI, NVMe and
  device-mapper drivers, and add the FALLOC_FL_WRITE_ZEROES and
  STATX_ATTR_WRITE_ZEROES_UNMAP support for ext4 and raw bdev devices.

  fallocate() is subsequently extended with the FALLOC_FL_WRITE_ZEROES
  flag. FALLOC_FL_WRITE_ZEROES zeroes a specified file range in such a
  way that subsequent writes to that range do not require further
  changes to the file mapping metadata. This flag is beneficial for
  subsequent pure overwriting within this range, as it can save on block
  allocation and, consequently, significant metadata changes"

This commit fixes several typos and grammatical issues across various

 - correct "glace" to "glance" in a comment in apple.c

 - change "unsucceesful" to "unsuccessful", "they blk-mq" to "the blk-mq",
 - fix "terminaed" to "terminated" and other grammar in fc.c
 - update "O's" to "0's" to clarify meaning in nvme.h
 - fix a function name reference in a comment in zns.c:
   *_transter_len() -> *_transfer_len().
 - fix sysfs_emit() output format in pci.c (replace x%08x with 0x%08x)

These changes improve the code readability and documentation consistency
across the NVMe driver.

71257925e8 nvme: fix misaccounting of nvme-mpath inflight I/O
Procedures for nvme-mpath IO accounting:

 1) initialize nvme_request and clear flags;
 2) set NVME_MPATH_IO_STATS and increase inflight counter when IO
    started;
 3) check NVME_MPATH_IO_STATS and decrease inflight counter when IO is
    done;

However, for the case nvme_fail_nonready_command(), both step 1) and 2)
are skipped, and if old nvme_request set NVME_MPATH_IO_STATS and then
request is reused, step 3) will still be executed, causing inflight I/O
counter to be negative.

Fix the problem by clearing nvme_request in nvme_fail_nonready_command().

1fc09f2961 nvme: revert the cross-controller atomic write size validation
This was originally added by commit 8695f060a0 ("nvme: all namespaces
in a subsystem must adhere to a common atomic write size") to check
the all controllers in a subsystem report the same atomic write size,
but the check wasn't quite correct and caused problems for devices
with multiple namespaces that report different LBA sizes.  Commit
f46d273449 ("nvme: fix atomic write size validation") tried to fix
this, but then caused problems for namespace rediscovery after a
format with an LBA size change that changes the AWUPF value.

This drops the validation and essentially reverts those two commits while
keeping the cleanup that went in between the two.  We'll need to figure
out how to properly check for the mouse trap that nvme left us, but for
now revert the check to keep devices working for users who couldn't care
less about the atomic write feature.

80d7762e0a nvme: fix inconsistent RCU list manipulation in nvme_ns_add_to_ctrl_list()
When inserting a namespace into the controller's namespace list, the
function uses list_add_rcu() when the namespace is inserted in the middle
of the list, but falls back to a regular list_add() when adding at the
head of the list.

This inconsistency could lead to race conditions during concurrent
access, as users might observe a partially updated list. Fix this by
consistently using list_add_rcu() in both code paths to ensure proper
RCU protection throughout the entire function.

block: introduce pi_tuple_size field in blk_integrity
Introduce a new pi_tuple_size field in struct blk_integrity to
explicitly represent the size (in bytes) of the protection information
(PI) tuple. This is a prep patch.
Add validation in blk_validate_integrity_limits() to ensure that
pi size matches the expected size for known checksum types and never
exceeds the pi_tuple_size.

ba806c9003 nvme: correctly account for namespace head reference counter
The blktests nvme/058 manifests an issue where the NVMe subsystem
kobject entry remains stale in sysfs, causing a failure during
subsequent NVMe module reloads[1]. Specifically, when attempting to
register a new NVMe subsystem, the driver encounters a kobejct name
collision because a stale kobject still exists. Though, please note
that nvme/058 doesn't report any failure and test case passes and
it's only during subsequent NVMe module reloads, the stale nvme sub-
system kobject entry in sysfs causes the observed symptom[1].

This issue stems from an imbalance in the get/put usage of the namespace
head (nshead) reference counter. The nshead holds a reference to the
associated NVMe subsystem. If the nshead reference is not properly
released, it prevents the cleanup of the subsystem's kobject, leaving
nvme subsystem stale entry behind in sysfs.

During the failure case, the last namespace path referencing a nshead
is removed, but the nshead reference was not released. This occurs
because the release logic currently only puts the nshead reference
when its state is LIVE. However, in configurations where ANA (Asymmetric
Namespace Access) is enabled, a namespace may be associated with an ANA
state that is neither optimized nor non-optimized. In this case, the
nshead may never transition to LIVE, and the corresponding nshead
reference is then never dropped. In fact nvme/058 associates some of
nvme namespaces to an inaccessible ANA state and with that nshead is
created but it's state is not transitioned to LIVE. So the current
logic would then causes nshead reference to be leaked for non-LIVE
states.

Another scenario, during namespace allocation, the driver first
allocates a nshead and then issues an Identify Namespace command. If
this command fails — which can happen in tests like nvme/058 that
rapidly enables and disables namespaces — we must release the reference
to the newly allocated nshead. However this reference release is
currently missing in the failure, causing a nshead reference leak.

To fix this, we now unconditionally release the nshead reference when
the last nvme path referencing to the nshead is removed, regardless of
the head’s state. Also during identify namespace failure case we now
properly release the nshead reference. So this ensures proper cleanup
of the nshead, and consequently, the NVMe subsystem and its associated
kobject.

This change prevents stale kobject entries from lingering in sysfs and
eliminates the module reload failures observed just after running
nvme/058.

f46d273449 nvme: fix atomic write size validation
Don't mix the namespace and controller values, and validate the
per-controller limit when probing the controller.  This avoid spurious
failures for controllers with namespaces that have different namespaces
with different logical block sizes, or report the per-namespace values
only for some namespaces.

It also fixes a missing queue_limits_cancel_update in an error path by
removing that error path.
