The driver previously used a waitqueue along with an explicit
request_done flag, but without proper barriers around request_done.
An earlier patch by Gui-Dong Han <hanguidong02@gmail.com> attempted
to fix this by adding the missing memory barriers. Rather than
adding the barriers, this patch replaces the waitqueue+flag with
a completion, which is designed for this exact purpose.

When we generate completion for SQ the opcode while being properly read
from ring buffer is ignored when written back to completion. Seems
to be a simple typo.

During reset, irdma_modify_qp() to error should be called to disconnect
the QP. Without this fix, if not preceded by irdma_modify_qp() to error, the
API call irdma_destroy_qp() gets stuck waiting for the QP refcount to go
to zero, because the cm_node associated with this QP isn't disconnected.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

Allocate array chunk->dmainfo.dmaaddrs using kvzalloc() to allow the

The hardware allows for an opaque CQ context field to be carried
over into CEQEs for the CQ. Previously, a pointer to the CQ was used
for this context. In the normal CQ destroy flow, the CEQ ring is
scrubbed to remove any preexisting CEQEs for the CQ that may not have
been processed yet so that the CQ structure is not dereferenced in the
CEQ ISR after the CQ has been freed.
However, in some cases, it is possible for a CEQE to be in flight in
HW even after the CQ destroy command completion is received, so it
could be missed during the scrub.
To protect against this, we can take advantage of the CQ table that
already exists and use the CQ ID for this context rather than a CQ
pointer.

The AH CQP command wait loop executes in an atomic context and was
using a fixed 1 ms delay. Since many AH create commands can complete
much faster than 1 ms, use poll_timeout_us_atomic with a 1 us delay.
Also, use the timeout value indicated during the capability exchange
rather than a hard-coded value.

irdma_net_event() should not dereference anything from "neigh" (alias
"ptr") until it has checked that the event is NETEVENT_NEIGH_UPDATE.
Other events come with different structures pointed to by "ptr" and they
may be smaller than struct neighbour.
Move the read of neigh->dev under the NETEVENT_NEIGH_UPDATE case.
The bug is mostly harmless, but it triggers KASAN on debug kernels:

CQ shadow area should not be updated at the end of a page (once every
64th CQ entry), except when CQ has no more CQEs. SW must also increase
the requested CQ size by 1 and make sure the CQ is not exactly one page
in size. This is to address a quirk in the hardware.

Failure to initialize info.create field to false in certain cases
was resulting in incorrect status code going to rdma-core when dereg_mr
failed during reset. To fix this, memset entire cqp_request->info
in irdma_alloc_and_get_cqp_request() function, so that this is not spread
all over the code.

Add a new RDMA virtual channel op during QP1 creation that allow the
Control Plane (CP) to virtualize a regular QP as QP1 on non-default
RDMA capable vPorts. Additionally, the CP will return the Qsets to use
on the ib_device of the vPort.

Now that the crc32c() library function directly takes advantage of
architecture-specific optimizations, it is unnecessary to go through the
crypto API. Just use crc32c(). This is much simpler, and it improves
performance due to eliminating the crypto API overhead.
Note that for crc32c the equivalent of crypto_shash_digest() is
cpu_to_le32(~crc32c(~0, ...)), considering that crypto_shash_digest()
had before and inversions as well as a cpu_to_le32() built-in. This
means that this driver is using u32 for fixed-endian types; this patch
does not try to fix that but rather just keep the exact same behavior.

Add a CQ table based loookup to allow quick search
for CQ pointer having CQ ID in case of CQ related
asynchrononous event. The table is implemented in a
similar fashion to QP table.
Also add a reference counters for CQ. This is to prevent
destroying CQ while an asynchronous event is being processed.
The memory resource table size is sized higher with this update,
and this table doesn't need to be physically contiguous, so use
a vzalloc vs kzalloc to allocate the table.

CQP completion statistics is read lockesly in irdma_wait_event and
irdma_check_cqp_progress while it can be updated in the completion
thread irdma_sc_ccq_get_cqe_info on another CPU as KCSAN reports.
Make completion statistics an atomic variable to reflect coherent updates
to it. This will also avoid load/store tearing logic bug potentially
possible by compiler optimizations.

Currently, artificial SW completions are generated for NOP wqes which can
generate unexpected completions with wr_id = 0. Skip the generation of
artificial completions for NOPs.

Refactor HW statistics which,
- Unifies HW statistics support for all HW generations.
- Unifies support of 32- and 64-bit counters.
- Removes duplicated code and simplifies implementation.
- Fixes roll-over handling.
- Removes unneeded last_hw_stats.
With new implementation, there is no separate handling and no separate
arrays for 32- and 64-bit counters (offsets, regs, values). Instead,
there is a HW stats map array for each HW revision, which defines
HW-specific width and location of each counter in the statistics buffer.
Once the statistics are gathered (either via CQP op, or by reading HW
registers), counter values are extracted from the statistics buffer using
the stats map and the delta between the last and new values is computed.
Finally, the counter values in rdma_hw_stats are incremented by those
deltas.
From the OS perspective, all the counters are 64-bit and their order in
rdma_hw_stats->value[] array, as well as in irdma_hw_stat_names[], is the
same for all HW gens. New statistics should always be added at the end.

The opcode written by HW, in the RQ CQE, is the
RoCEv2/iWARP protocol opcode from the received
packet and not the SW opcode as currently assumed.
Fix this by returning the raw operation type and
queue type in the CQE to irdma_process_cqe and add
2 helpers set_ib_wc_op_sq set_ib_wc_op_rq to map
IRDMA HW op types to IB op types.
Note that for iWARP, only Write with Immediate is
supported so the opcode can only be IB_WC_RECV_RDMA_WITH_IMM
when there is immediate data present.

A number of asynchronous event (AE) ids were not aligned to the
correct flush_code and event_type. Fix these up so that the
correct IBV error and event codes are returned to application.
Also, add handling for new AE ids like IRDMA_AE_INVALID_REQUEST to
return the correct WC error code.

HW flushes after QP in error state is not reliable. This can lead to
application hang waiting on a completion for outstanding WRs. Implement a
SW mechanism to generate completions for any outstanding WR's after the QP
is modified to error.
This is accomplished by starting a delayed worker after the QP is modified
to error and the HW flush is performed. The worker will generate
completions that will be returned to the application when it polls the
CQ. This mechanism only applies to Kernel applications.

For some net events in irdma_net_event notifier, the netdev can be NULL
which will cause a crash in rdma_vlan_dev_real_dev. Fix this by moving
all processing to the NETEVENT_NEIGH_UPDATE case where the netdev is
guaranteed to not be NULL.

Completion events (CEs) are lost if the application is allowed to arm the
CQ more than two times when no new CE for this CQ has been generated by
the HW.
Check if arming has been done for the CQ and if not, arm the CQ for any
event otherwise promote to arm the CQ for any event only when the last arm
event was solicited.

'pchunk->bitmapbuf' is a bitmap. Its size (in number of bits) is stored in
'pchunk->sizeofbitmap'.
When it is allocated, the size (in bytes) is computed by:
size_in_bits >> 3
There are 2 issues (numbers bellow assume that longs are 64 bits):
- there is no guarantee here that 'pchunk->bitmapmem.size' is modulo
BITS_PER_LONG but bitmaps are stored as longs
(sizeofbitmap=8 bits will only allocate 1 byte, instead of 8 (1 long))
- the number of bytes is computed with a shift, not a round up, so we
may allocate less memory than needed
(sizeofbitmap=65 bits will only allocate 8 bytes (i.e. 1 long), when 2
longs are needed = 16 bytes)
Fix both issues by using 'bitmap_zalloc()' and remove the useless
'bitmapmem' from 'struct irdma_chunk'.
While at it, remove some useless NULL test before calling
kfree/bitmap_free.

This leads CQP failures such as below:

Coverity reports a signed 32-bit overflow on "1 << pprm->pble_shift" when
used expression to compute bits_needed that expects 64bit, unsigned.

The level1 PBL info address is stored as u64. This requires casting
through a uinptr_t before used as a pointer type.
And this leads to sparse warning such as this when uinptr_t is missing:
