The ioctl handlers only test REMOVE_PENDING before entering
mtip_hw_ioctl(). Removal can set that bit immediately afterwards and free
dd->port in mtip_hw_exit() while an ioctl still dereferences it. An already
open block device can reach the handlers while del_gendisk() is in
progress.
Serialize both native and compat ioctls with removal. Set REMOVE_PENDING
before taking the mutex so new callers fail after an in-flight ioctl has
drained, and hold the mutex until the port has been torn down.

If service thread creation fails after device_add_disk() succeeds,
mtip_block_initialize() calls del_gendisk() and then falls through to
put_disk(). Since mtip32xx uses .free_disk to free struct driver_data,
put_disk() can release dd on the added-disk path.
The same unwind then continues to use dd for blk_mq_free_tag_set() and
mtip_hw_exit(), and mtip_pci_probe() can later free dd again. This can
cause a use-after-free and double free.
Track whether the disk was added in the current initialization call.
For the post-add service-thread failure path, remove the disk, release
the local hardware resources, and return without dropping the final disk
reference. The probe error path can then finish its cleanup and call
put_disk() after it is done using dd. Keep the pre-add path using
put_disk() before blk_mq_free_tag_set(), and clear dd->disk so the outer
probe cleanup frees dd directly.

Instances are happier that way and it makes more sense anyway -
the only part of the result that is related to partition we are given
is the start sector, and that has been filled in by the caller.
Everything else is a function of the disk. Only one instance
(DASD) is ever looking at anything other than bdev->bd_disk and
that one is trivial to adjust.

The dma_map_sg() can fail and, in case of failure, returns 0. If it
fails, mtip_hw_submit_io() returns an error.
The dma_unmap_sg() requires the nents parameter to be the same as the
one passed to dma_map_sg(). This patch saves the nents in
command->scatter_ents.

pcim_iounmap_regions() is deprecated. Moreover, it is not necessary to call
it in the driver's remove() function or if probe() fails, since it does

BLK_MQ_F_SHOULD_MERGE is set for all tag_sets except those that purely
process passthrough commands (bsg-lib, ufs tmf, various nvme admin
queues) and thus don't even check the flag. Remove it to simplify the
driver interface.

no_llseek had been defined to NULL two years ago, in commit 868941b144
("fs: remove no_llseek")
To quote that commit,
At -rc1 we'll need do a mechanical removal of no_llseek -

would do it.
Unfortunately, that hadn't been done. Linus, could you do that now, so

Since the debugfs_create_dir() never returns a null pointer, checking
the return value for a null pointer is redundant. Since
debugfs_create_file() can deal with a ERR_PTR() style pointer, drop
the check. Since mtip_hw_debugfs_init does not pay attention to the
return value, its return type can be changed to void.

Move the add_random flag into the queue_limits feature field so that it
can be set atomically with the queue frozen.
Note that this also removes code from dm to clear the flag based on
the underlying devices, which can't be reached as dm devices will
always start out without the flag set.

Move the nonrot flag into the queue_limits feature field so that it can
be set atomically with the queue frozen.
Use the chance to switch to defaulting to non-rotational and require
the driver to opt into rotational, which matches the polarity of the
sysfs interface.
For the z2ram, ps3vram, 2x memstick, ubiblock and dcssblk the new
rotational flag is not set as they clearly are not rotational despite
this being a behavior change. There are some other drivers that
unconditionally set the rotational flag to keep the existing behavior
as they arguably can be used on rotational devices even if that is
probably not their main use today (e.g. virtio_blk and drbd).
The flag is automatically inherited in blk_stack_limits matching the
existing behavior in dm and md.

The only overlap between the block open flags mapped into the fmode_t and
other uses of fmode_t are FMODE_READ and FMODE_WRITE. Define a new
blk_mode_t instead for use in blkdev_get_by_{dev,path}, ->open and
->ioctl and stop abusing fmode_t.

With new API blk_mq_is_reserved_rq() we can tell if a request is from
the reserved pool, so stop passing 'reserved' arg. There is actually
only a single user of that arg for all the callback implementations, which
can use blk_mq_is_reserved_rq() instead.
This will also allow us to stop passing the same 'reserved' around the
blk-mq iter functions next.

Set the queue dying flag and call blk_mq_exit_queue from del_gendisk for
all disks that do not have separately allocated queues, and thus remove
the need to call blk_cleanup_queue for them.
Rename blk_cleanup_disk to blk_mq_destroy_queue to make it clear that
this function is intended only for separately allocated blk-mq queues.
This saves an extra queue freeze for devices without a separately
allocated queue.

q->queue_hw_ctx is really one blk-mq internal structure for retrieving
hctx via its index, not supposed to be used by drivers. Meantime drivers
can get the tags structure easily from tagset.

Various block drivers call blk_set_queue_dying to mark a disk as dead due
to surprise removal events, but since commit 8e141f9eb8 that doesn't
work given that the GD_DEAD flag needs to be set to stop I/O.
Replace the driver calls to blk_set_queue_dying with a new (and properly
documented) blk_mark_disk_dead API, and fold blk_set_queue_dying into the
only remaining caller.

The .suspend() and .resume() methods are only called after the .probe()
method (mtip_pci_probe()) has set the drvdata and returned success.
Therefore, if we get to mtip_pci_suspend() or mtip_pci_resume(), the
drvdata must be valid. Drop the unnecessary checking.

Move the call to blk_should_fake_timeout out of blk_mq_complete_request
and into the drivers, skipping call sites that are obvious error
handlers, and remove the now superflous blk_mq_force_complete_rq helper.
This ensures we don't keep injecting errors into completions that just
terminate the Linux request after the hardware has been reset or the
command has been aborted.
