fb87c88272 mm: change to return bool for pudp_test_and_clear_young()
The pudp_test_and_clear_young() is used to clear the young flag, returning
whether the young flag was set for this PUD entry. Change the return type
to bool to make the intention clearer.

4894e2fb7b powerpc/64s: Kill the unused argument of exit_lazy_flush_tlb
In previous patch we removed the only caller of exit_lazy_flush_tlb()
which was passing always_flush = false in it's second argument.

With that gone, all the callers of exit_lazy_flush_tlb() are local to
radix_pgtable.c and there is no need of an additional argument.

This patch does the required cleanup. There should not be any
functionality change in this patch.

bf7c1497d2 powerpc/64s: Move serialize_against_pte_lookup() to hash_pgtable.c
Originally,
commit fa4531f753 ("powerpc/mm: Don't send IPI to all cpus on THP updates")
introduced serialize_against_pte_lookup() call for both Radix and Hash.

However below commit fixed the race with Radix
commit 70cbc3cc78 ("mm: gup: fix the fast GUP race against THP collapse")

And therefore following commit removed the
serialize_against_pte_lookup() call from radix_pgtable.c
commit bedf034169
("powerpc/64s/radix: don't need to broadcast IPI for radix pmd collapse flush")

Now since serialize_against_pte_lookup() only gets called from
hash__pmdp_collapse_flush(), thus move the related functions to
hash_pgtable.c

Hence this patch:
- moves serialize_against_pte_lookup() from radix_pgtable.c to hash_pgtable.c
- removes the radix specific calls from do_serialize()
- renames do_serialize() to do_nothing().

There should not be any functionality change in this patch.

bbcbf045d6 powerpc/64s: Fix unmap race with PMD migration entries
The following race is possible with migration swap entries or
device-private THP entries. e.g. when move_pages is called on a PMD THP

641d47d4c9 powerpc/mm: support page table check
On creation and clearing of a page table mapping, instrument such calls by
invoking page_table_check_pte_set and page_table_check_pte_clear
respectively. These calls serve as a sanity check against illegal
mappings.

Enable ARCH_SUPPORTS_PAGE_TABLE_CHECK on powerpc, except when HUGETLB_PAGE
is enabled (powerpc has some weirdness in how it implements
set_huge_pte_at(), which may require some further work).

2360f523a4 powerpc/mm: use set_pte_at_unchecked() for internal usages
In the new set_ptes() API, set_pte_at() (a special case of set_ptes()) is
intended to be instrumented by the page table check facility. There are
however several other routines that constitute the API for setting page
table entries, including set_pmd_at() among others. Such routines are
themselves implemented in terms of set_ptes_at().

A future patch providing support for page table checking on powerpc must
take care to avoid duplicate calls to page_table_check_p{te,md,ud}_set().
Allow for assignment of pte entries without instrumentation through the
set_pte_at_unchecked() routine introduced in this patch.

Cause API-facing routines that call set_pte_at() to instead call
set_pte_at_unchecked(), which will remain uninstrumented by page table
check. set_ptes() is itself implemented by calls to __set_pte_at(), so
this eliminates redundant code.

78ddaa358e mm: change vm_get_page_prot() to accept vm_flags_t argument
Patch series "use vm_flags_t consistently".

The VMA flags field vma->vm_flags is of type vm_flags_t. Right now this
is exactly equivalent to unsigned long, but it should not be assumed to
be.

Much code that references vma->vm_flags already correctly uses vm_flags_t,
but a fairly large chunk of code simply uses unsigned long and assumes
that the two are equivalent.

This series corrects that and has us use vm_flags_t consistently.

This series is motivated by the desire to, in a future series, adjust
vm_flags_t to be a u64 regardless of whether the kernel is 32-bit or
64-bit in order to deal with the VMA flag exhaustion issue and avoid all
the various problems that arise from it (being unable to use certain
features in 32-bit, being unable to add new flags except for 64-bit, etc.)

This is therefore a critical first step towards that goal. At any rate,
using the correct type is of value regardless.

We additionally take the opportunity to refer to VMA flags as vm_flags
where possible to make clear what we're referring to.

Overall, this series does not introduce any functional change.

This patch (of 3):

We abstract the type of the VMA flags to vm_flags_t, however in may places
it is simply assumed this is unsigned long, which is simply incorrect.

At the moment this is simply an incongruity, however in future we plan to
change this type and therefore this change is a critical requirement for
doing so.

Overall, this patch does not introduce any functional change.

d82d3bf411 mm: pass mm down to pagetable_{pte,pmd}_ctor
Patch series "Always call constructor for kernel page tables", v2.

There has been much confusion around exactly when page table
constructors/destructors (pagetable_*_[cd]tor) are supposed to be called.
They were initially introduced for user PTEs only (to support split page
table locks), then at the PMD level for the same purpose. Accounting was
added later on, starting at the PTE level and then moving to higher levels
(PMD, PUD). Finally, with my earlier series "Account page tables at all
levels" [1], the ctor/dtor is run for all levels, all the way to PGD.

I thought this was the end of the story, and it hopefully is for user
pgtables, but I was wrong for what concerns kernel pgtables. The current
situation there makes very little sense:

* At the PTE level, the ctor/dtor is not called (at least in the generic
implementation). Specific helpers are used for kernel pgtables at this
level (pte_{alloc,free}_kernel()) and those have never called the
ctor/dtor, most likely because they were initially irrelevant in the
  kernel case.

* At all other levels, the ctor/dtor is normally called. This is
potentially wasteful at the PMD level (more on that later).

This series aims to ensure that the ctor/dtor is always called for kernel
pgtables, as it already is for user pgtables. Besides consistency, the
main motivation is to guarantee that ctor/dtor hooks are systematically
called; this makes it possible to insert hooks to protect page tables [2],
for instance. There is however an extra challenge: split locks are not
used for kernel pgtables, and it would therefore be wasteful to initialise
them (ptlock_init()).

It is worth clarifying exactly when split locks are used. They clearly
are for user pgtables, but as illustrated in commit 61444cde91 ("ARM:
8591/1: mm: use fully constructed struct pages for EFI pgd allocations"),
they also are for special page tables like efi_mm. The one case where
split locks are definitely unused is pgtables owned by init_mm; this is
consistent with the behaviour of apply_to_pte_range().

The approach chosen in this series is therefore to pass the mm associated
to the pgtables being constructed to pagetable_{pte,pmd}_ctor() (patch 1),
and skip ptlock_init() if mm == &init_mm (patch 3 and 7). This makes it
possible to call the PTE ctor/dtor from pte_{alloc,free}_kernel() without
unintended consequences (patch 3). As a result the accounting functions
are now called at all levels for kernel pgtables, and split locks are
never initialised.

In configurations where ptlocks are dynamically allocated (32-bit,
PREEMPT_RT, etc.) and ARCH_ENABLE_SPLIT_PMD_PTLOCK is selected, this
series results in the removal of a kmem_cache allocation for every kernel
PMD. Additionally, for certain architectures that do not use
<asm-generic/pgalloc.h> such as s390, the same optimisation occurs at the
PTE level.

===

Things get more complicated when it comes to special pgtable allocators
(patch 8-12). All architectures need such allocators to create initial
kernel pgtables; we are not concerned with those as the ctor cannot be
called so early in the boot sequence. However, those allocators may also
be used later in the boot sequence or during normal operations. There are
two main use-cases:

1. Mapping EFI memory: efi_mm (arm, arm64, riscv)
2. arch_add_memory(): init_mm

The ctor is already explicitly run (at the PTE/PMD level) in the first
case, as required for pgtables that are not associated with init_mm.
However the same allocators may also be used for the second use-case (or
others), and this is where it gets messy. Patch 1 calls the ctor with
NULL as mm in those situations, as the actual mm isn't available.
Practically this means that ptlocks will be unconditionally initialised.
This is fine on arm - create_mapping_late() is only used for the EFI
mapping. On arm64, __create_pgd_mapping() is also used by
arch_add_memory(); patch 8/9/11 ensure that ctors are called at all levels
with the appropriate mm. The situation is similar on riscv, but
propagating the mm down to the ctor would require significant refactoring.
Since they are already called unconditionally, this series leaves riscv
no worse off - patch 10 adds comments to clarify the situation.

From a cursory look at other architectures implementing arch_add_memory(),
s390 and x86 may also need a similar treatment to add constructor calls.
This is to be taken care of in a future version or as a follow-up.

===

The complications in those special pgtable allocators beg the question:
does it really make sense to treat efi_mm and init_mm differently in e.g.
apply_to_pte_range()? Maybe what we really need is a way to tell if an mm
corresponds to user memory or not, and never use split locks for non-user
mm's. Feedback and suggestions welcome!

This patch (of 12):

In preparation for calling constructors for all kernel page tables while
eliding unnecessary ptlock initialisation, let's pass down the associated
mm to the PTE/PMD level ctors. (These are the two levels where ptlocks
are used.)

In most cases the mm is already around at the point of calling the ctor so
we simply pass it down. This is however not the case for special page
table allocators:

* arch/arm/mm/mmu.c
* arch/arm64/mm/mmu.c
* arch/riscv/mm/init.c

In those cases, the page tables being allocated are either for standard
kernel memory (init_mm) or special page directories, which may not be
associated to any mm. For now let's pass NULL as mm; this will be refined
where possible in future patches.

No functional change in this patch.

5a0fcb0ef5 cxl: Remove driver
Remove the cxl driver that provides support for the IBM Coherent
Accelerator Processor Interface. Revert or clean up associated code in
arch/powerpc that is no longer necessary.

cxl has received minimal maintenance for several years, and is not
supported on the Power10 processor. We aren't aware of any users who are
likely to be using recent kernels.

Thanks to Mikey Neuling, Ian Munsie, Daniel Axtens, Frederic Barrat,

3d45a3d0d2 powerpc: Define config option for processors with broadcast TLBIE
Power ISA v3.0 (and later) implementations in the Linux Compliancy
Subset and lower are not required to implement broadcast TLBIE, and in
fact Microwatt doesn't.

To avoid the need to specify "disable_tlbie" on the kernel command
line on SMP Microwatt systems, this defines a config option that
asserts that the platform implements broadcast TLBIE. This option is
selected by the pseries and powernv platforms, but not by microwatt.
Note that this option is only relevant when the radix MMU is being
used, so platforms without a radix MMU don't need it.

c6f239796b mm/memblock: add memblock_alloc_or_panic interface
Before SLUB initialization, various subsystems used memblock_alloc to
allocate memory. In most cases, when memory allocation fails, an
immediate panic is required. To simplify this behavior and reduce
repetitive checks, introduce `memblock_alloc_or_panic`. This function
ensures that memory allocation failures result in a panic automatically,
improving code readability and consistency across subsystems that require
this behavior.

db6b435d73 mm: pgtable: introduce pagetable_dtor()
The pagetable_p*_dtor() are exactly the same except for the handling of
ptlock. If we make ptlock_free() handle the case where ptdesc->ptl is
NULL and remove VM_BUG_ON_PAGE() from pmd_ptlock_free(), we can unify
pagetable_p*_dtor() into one function.  Let's introduce pagetable_dtor()
to do this.

Later, pagetable_dtor() will be moved to tlb_remove_ptdesc(), so that
ptlock and page table pages can be freed together (regardless of whether
RCU is used). This prevents the use-after-free problem where the ptlock
is freed immediately but the page table pages is freed later via RCU.

b5fbf7e2c6 book3s64/radix: Refactoring common kfence related functions
Both radix and hash on book3s requires to detect if kfence
early init is enabled or not. Hash needs to disable kfence
if early init is not enabled because with kfence the linear map is
mapped using PAGE_SIZE rather than 16M mapping.
We don't support multiple page sizes for slb entry used for kernel
linear map in book3s64.

This patch refactors out the common functions required to detect kfence
early init is enabled or not.

4dd7724f02 mm/powerpc: add missing pud helpers
Some new helpers will be needed for pud entry updates soon. Introduce
these helpers by referencing the pmd ones. Namely:

  - pudp_invalidate(): this helper invalidates a huge pud before a split
happens, so that the invalidated pud entry will make sure no race will
happen (either with software, like a concurrent zap, or hardware, like
  a/d bit lost).

  - pud_modify(): this helper applies a new pgprot to an existing huge pud
  mapping.

For more information on why we need these two helpers, please refer to the
corresponding pmd helpers in the mprotect() code path.

57fb15c32f powerpc/64s: use contiguous PMD/PUD instead of HUGEPD
On book3s/64, the only user of hugepd is hash in 4k mode.

All other setups (hash-64, radix-4, radix-64) use leaf PMD/PUD.

Rework hash-4k to use contiguous PMD and PUD instead.

In that setup there are only two huge page sizes: 16M and 16G.

16M sits at PMD level and 16G at PUD level.

pte_update doesn't know page size, lets use the same trick as
hpte_need_flush() to get page size from segment properties. That's not
the most efficient way but let's do that until callers of pte_update()
provide page size instead of just a huge flag.

3a5a8d343e mm: fix race between __split_huge_pmd_locked() and GUP-fast
__split_huge_pmd_locked() can be called for a present THP, devmap or
(non-present) migration entry. It calls pmdp_invalidate() unconditionally
on the pmdp and only determines if it is present or not based on the
returned old pmd. This is a problem for the migration entry case because
pmd_mkinvalid(), called by pmdp_invalidate() must only be called for a
present pmd.

On arm64 at least, pmd_mkinvalid() will mark the pmd such that any future
call to pmd_present() will return true. And therefore any lockless
pgtable walker could see the migration entry pmd in this state and start
interpretting the fields as if it were present, leading to BadThings (TM).
GUP-fast appears to be one such lockless pgtable walker.

x86 does not suffer the above problem, but instead pmd_mkinvalid() will
corrupt the offset field of the swap entry within the swap pte. See link
below for discussion of that problem.

Fix all of this by only calling pmdp_invalidate() for a present pmd. And
for good measure let's add a warning to all implementations of
pmdp_invalidate[_ad](). I've manually reviewed all other
pmdp_invalidate[_ad]() call sites and believe all others to be conformant.

This is a theoretical bug found during code review. I don't have any test
case to trigger it in practice.

b1fba034a6 powerpc: Support execute-only on all powerpc
Introduce PAGE_EXECONLY_X macro which provides exec-only rights.
The _X may be seen as redundant with the EXECONLY but it helps
keep consistency, all macros having the EXEC right have _X.

And put it next to PAGE_NONE as PAGE_EXECONLY_X is
somehow PAGE_NONE + EXEC just like all other SOMETHING_X are
just SOMETHING + EXEC.

On book3s/64 PAGE_EXECONLY becomes PAGE_READONLY_X.

On book3s/64, as PAGE_EXECONLY is only valid for Radix add
VM_READ flag in vm_get_page_prot() for non-Radix.

And update access_error() so that a non exec fault on a VM_EXEC only
mapping is always invalid, even when the underlying layer don't
always generate a fault for that.

For 8xx, set PAGE_EXECONLY_X as _PAGE_NA | _PAGE_EXEC.
For others, only set it as just _PAGE_EXEC

With that change, 8xx, e500 and 44x fully honor execute-only
protection.

On 40x that is a partial implementation of execute-only. The
implementation won't be complete because once a TLB has been loaded
via the Instruction TLB miss handler, it will be possible to read
the page. But at least it can't be read unless it is executed first.

On 603 MMU, TLB missed are handled by SW and there are separate
DTLB and ITLB. Execute-only is therefore now supported by not loading
DTLB when read access is not permitted.

On hash (604) MMU it is more tricky because hash table is common to
load/store and execute. Nevertheless it is still possible to check
whether _PAGE_READ is set before loading hash table for a load/store
access. At least it can't be read unless it is executed first.

27af67f356 powerpc/book3s64/mm: enable transparent pud hugepage
This is enabled only with radix translation and 1G hugepage size. This
will be used with devdax device memory with a namespace alignment of 1G.

Anon transparent hugepage is not supported even though we do have helpers
checking pud_trans_huge(). We should never find that return true. The
only expected pte bit combination is _PAGE_PTE | _PAGE_DEVMAP.

Some of the helpers are never expected to get called on hash translation
and hence is marked to call BUG() in such a case.

54f30b83fe powerpc: address missing-prototypes warnings
There are a few warnings in powerpc64 defconfig builds after -Wmissing-prototypes
gets promoted from W=1 to the default warning set:

arch/powerpc/mm/book3s64/pgtable.c:422:6: error: no previous prototype for 'arch_report_meminfo' [-Werror=missing-prototypes]
arch/powerpc/platforms/cell/ras.c:275:5: error: no previous prototype for 'cbe_sysreset_hack' [-Werror=missing-prototypes]
arch/powerpc/platforms/cell/spu_manage.c:29:21: error: no previous prototype for 'spu_devnode' [-Werror=missing-prototypes]
arch/powerpc/platforms/pasemi/time.c:12:17: error: no previous prototype for 'pas_get_boot_time' [-Werror=missing-prototypes]
arch/powerpc/platforms/powermac/feature.c:1532:13: error: no previous prototype for 'g5_phy_disable_cpu1' [-Werror=missing-prototypes]
arch/powerpc/platforms/86xx/pic.c:28:13: error: no previous prototype for 'mpc86xx_init_irq' [-Werror=missing-prototypes]
drivers/pci/pci-sysfs.c:936:13: error: no previous prototype for 'pci_adjust_legacy_attr' [-Werror=missing-prototypes]

Address these by including the right header files or marking the
functions static. The audit.c one is a bit tricky since compat_audit.h
cannot include regular kernel headers tht have conflicting types on
32-bit powerpc.

395cac7752 powerpc/mm: Support execute-only memory on the Radix MMU
Add support for execute-only memory (XOM) for the Radix MMU by using an
execute-only mapping, as opposed to the RX mapping used by powerpc's
other MMUs.

The Hash MMU already supports XOM through the execute-only pkey,
which is a separate mechanism shared with x86. A PROT_EXEC-only mapping
will map to RX, and then the pkey will be applied on top of it.

mmap() and mprotect() consumers in userspace should observe the same
behaviour on Hash and Radix despite the differences in implementation.

Replacing the vma_is_accessible() check in access_error() with a read
check should be functionally equivalent for non-Radix MMUs, since it
follows write and execute checks. For Radix, the change enables
detecting faults on execute-only mappings where vma_is_accessible() would
return true.

53f45ecc9c powerpc/mm: Move setting PTE specific flags to pfn_pmd()
powerpc used to set the PTE specific flags in set_pte_at(). That is
different from other architectures. To be consistent with other

379c926d63 ("powerpc/mm: move setting pte specific flags to

That commit didn't do the same for pfn_pmd() because we expect
pmd_mkhuge() to do that. But as per Linus that is a bad rule:

The rule that you must use "pmd_mkhuge()" seems _completely_ wrong.
The only valid use to ever make a pmd out of a pfn is to make a
  huge-page.

Hence update pfn_pmd() to set _PAGE_PTE.

we need to ensure that new kernels get kexec'd with a reset value
of AMR/IAMR. For ex: the new kernel can use key 0 for kernel mapping and the old
AMR value prevents access to key 0.

This patch also removes reset if IAMR and AMOR in kexec_sequence. Reset of AMOR
is not needed and the IAMR reset is partial (it doesn't do the reset
on secondary cpus) and is redundant with this patch.

This occurs when unplug is attempted for such memory which has
been mapped using memblock pages as part of early kernel page
table setup. We wouldn't have initialized the PMD or PTE fragment
count for those PMD or PTE pages.

This can be fixed by allocating memory in PAGE_SIZE granularity
during early page table allocation. This makes sure a specific
page is not shared for another memblock allocation and we can
free them correctly on removing page-table pages.

Since we now do PAGE_SIZE allocations for both PUD table and
PMD table (Note that PTE table allocation is already of PAGE_SIZE),
we end up allocating more memory for the same amount of system RAM.
Here is a comparision of how much more we need for a 64T and 2G
system after this patch:

1. 64T system
-------------
64T RAM would need 64G for vmemmap with struct page size being 64B.

128 PUD tables for 64T memory (1G mappings)
1 PUD table and 64 PMD tables for 64G vmemmap (2M mappings)

With default PUD[PMD]_TABLE_SIZE(4K), (128+1+64)*4K=772K
With PAGE_SIZE(64K) table allocations, (128+1+64)*64K=12352K

2. 2G system
------------
2G RAM would need 2M for vmemmap with struct page size being 64B.

1 PUD table for 2G memory (1G mapping)
1 PUD table and 1 PMD table for 2M vmemmap (2M mappings)

With default PUD[PMD]_TABLE_SIZE(4K), (1+1+1)*4K=12K
With new PAGE_SIZE(64K) table allocations, (1+1+1)*64K=192K

18594f9b8c powerpc/64s/radix: Don't prefetch DAR in update_mmu_cache
The idea behind this prefetch was to kick off a page table walk before
returning from the fault, getting some pipelining advantage.

But this never showed up any noticable performance advantage, and in
fact with KUAP the prefetches are actually blocked and cause some
kind of micro-architectural fault. Removing this improves page fault
microbenchmark performance by about 9%.

75358ea359 powerpc/mm/book3s64: Fix MADV_DONTNEED and parallel page fault race
MADV_DONTNEED holds mmap_sem in read mode and that implies a
parallel page fault is possible and the kernel can end up with a level 1 PTE
entry (THP entry) converted to a level 0 PTE entry without flushing
the THP TLB entry.

Most architectures including POWER have issues with kernel instantiating a level
0 PTE entry while holding level 1 TLB entries.

The code sequence I am looking at is

down_read(mmap_sem) down_read(mmap_sem)

zap_pmd_range()
 zap_huge_pmd()
  pmd lock held
  pmd_cleared
  table details added to mmu_gather
  pmd_unlock()
insert a level 0 PTE entry()

tlb_finish_mmu().

Fix this by forcing a tlb flush before releasing pmd lock if this is
not a fullmm invalidate. We can safely skip this invalidate for
task exit case (fullmm invalidate) because in that case we are sure
there can be no parallel fault handlers.

This do change the Qemu guest RAM del/unplug time as below

128 core, 496GB guest:

Without patch:
munmap start: timer = 196449 ms, PID=6681
munmap finish: timer = 196488 ms, PID=6681 - delta = 39ms

With patch:
munmap start: timer = 196345 ms, PID=6879
munmap finish: timer = 196714 ms, PID=6879 - delta = 369ms

e21dfbf013 powerpc/mm/book3s64: Avoid sending IPI on clearing PMD
Now that all the lockless page table walk is careful w.r.t the PTE
address returned, we can now revert
commit: 13bd817bb8 ("powerpc/thp: Serialize pmd clear against a linux page table walk.")

We also drop the equivalent IPI from other pte updates routines. We still keep
IPI in hash pmdp collapse and that is to take care of parallel hash page table
insert. The radix pmdp collapse flush can possibly be removed once I am sure
generic code doesn't have the any expectations around parallel gup walk.

This speeds up Qemu guest RAM del/unplug time as below

128 core, 496GB guest:

Without patch:
munmap start: timer = 13162 ms, PID=7684
munmap finish: timer = 95312 ms, PID=7684 - delta = 82150 ms

With patch:
munmap start: timer = 196449 ms, PID=6681
munmap finish: timer = 196488 ms, PID=6681 - delta = 39ms
