RDMA/mlx4: Inline mlx4_ib_get_cq_umem into callers

Inline the mlx4_ib_get_cq_umem helper function into its two call sites
(mlx4_ib_create_cq and mlx4_alloc_resize_umem) to prepare for the
transition to modern CQ creation interface.
